This study was conducted to examine Medical Aid case managers’ perceptions regarding the scope of underutilization of healthcare services within the Medical Aid system, the criteria for identifying individuals underutilizing the system, and the factors influencing underutilization. Focus group discussions (FGDs) were conducted with Medical Aid service practitioners and civic organizations to explore the characteristics of underutilization, contributing factors, and potential solutions. The findings indicated that while participants commonly acknowledged the presence of individuals underutilizing the system, their views varied concerning the scale and extent. They also highlighted the absence of clear criteria to identify such individuals, which complicates the process of identifying them. Underutilization of healthcare by Medical Aid beneficiaries was found to result from a complex interplay of personal, socio-environmental, and institutional factors. In particular, many beneficiaries were perceived to have difficulties in voluntarily accessing healthcare services, underscoring the need for tailored interventions. This study provides evidence-based directions and a foundation for the effective management of individuals underutilizing the healthcare system, and contributes to laying the groundwork for future policies aimed at improving healthcare access for Medical Aid beneficiaries.

이 연구는 의료급여제도 내 과소이용 문제의 범위, 과소이용자 식별 기준의 존재 여부, 과소이용에 영향을 미 치는 요인에 대해 의료급여 사례관리자의 인식을 파악하기 위해 수행되었다. 의료급여 수급자의 의료서비스 과소이용 특성, 영향 요인, 가능한 해결 방안을 탐색하고자 의료급여 실무 담당자와 시민단체 활동가를 대상으로 포커스 그룹 토론(FGD)을 실시하였다. 연구 결과, 참여자들은 과소이용자의 존재를 공통적으로 인식하고 있었으나 그 규모나 범위 에 대해서는 시각 차이를 보였다. 또한 명확한 판단 기준의 부재로 인해 과소이용자 선별에 어려움이 있음을 지적하 였다. 의료급여 수급권자의 과소의료이용은 개인적, 사회 환경적, 제도적 요인이 복합적으로 작용한 결과로 나타났다. 특히 많은 의료급여 수급자들이 자발적으로 의료를 이용하기 어려운 특성을 지니고 있어 맞춤형 개입의 필요성이 확 인되었다. 이 연구는 의료급여 수급자의 과소 의료이용자에 대한 효과적인 관리 방향성과 근거를 제시함으로써, 향후 이들의 의료 접근성을 개선하기 위한 정책적 기초를 마련하는 데 의의가 있다.
