The purpose of this study was to examine the effects of music listening on pain, anxiety, and vital signs to the patients undergoing burn dressing changes. This study used a quasi-experimental study of nonequivalent control group pretest post test design. Total of 32 adult burn patients admitted to a tertiary hospital were collected by convenience sampling with 15 patients in the control group and 17 patients in the experimental group who listened to the music using headphones of CD player during dressing changes for three consecutive days. For data analysis, χ2-test, fisher's test, t-test, and repeated measures of ANOVA were used. There was an interaction effect in the pain score between experimental and control group. Anxiety score had interaction, group, and time effect between experimental and control group. Systolic blood pressure and pulse showed interaction effect between groups and time and respiration showed only group effect. These findings indicated that music listening is a valuable noninvasive intervention for the relieving pain and anxiety and stabilizing the vital signs of burn patients. Therefore, we recommend more clinical application of music listening to the various populations in different settings.

이 연구는 선호 음악감상이 화상환자의 드레싱교환으로 발생하는 통증, 불안, 그리고 활력징후에 어떤 영향을 미치는 지를 검증하기위해 시도되었다. 연구방법은 비동등성 대조군 전․후 시차 설계의 유사 실험연구이며, 대학병원에 입원한 화상환자 32명을 대상으로 실험군과 대조군에 각각 17명과 15명이 포함되었다. 실험군은 음악선호 질문지에 따라 확인된 음악을 CD 플레이어를 통해 드레싱 3일째부터 3일간 연속으로 듣게 하였으며, 음악감상에 제공된 시간은 드레싱 전후로 약 30분이다. 자료분석은 χ2-test, fisher's test, t-test, 그리고 반복측정분산분석을 사용하였다. 실험결과 통증은 실험군과 대조군에서 상호작용효과가 나타났으며, 불안은 상호작용과 그룹, 시간 효과가 모두 나타났다. 활력징후에서 수축기 혈압과 맥박은 상호작용효과가 나타났으며, 호흡은 그룹효과만 나타났다. 이상의 결과를 볼 때 선호 음악감상은 화상환자의 통증과 불안을 감소시키고 활력징후를 안정화시키는 효과적인 비 침습적 중재인 것으로 나타났다. 따라서 다른 환자 군에게도 음악감상의 효과에 대한 연구를 수행해 보기를 제안한다.
