This study aims to identify the structural causes underlying the repeated suspension and resumption of inter-Korean health cooperation over the past two decades, despite the expansion of agendas and technical expertise through inter-Korean summits, sectoral committees, and working-level consultations, and to propose legal and institutional directions for designing a sustainable cooperation framework in the future. The analysis demonstrates that the repeated breakdown of inter-Korean health cooperation cannot be attributed to policy failure during specific political periods or administrations. Rather, it results from the interaction of three structural mechanisms: (1) a support-oriented, one-directional cooperation structure that failed to accumulate the policy-based trust necessary for a transition toward cooperation grounded in mutual responsibility; (2) the quantitative expansion of agreements without corresponding institutional durability or implementation-support mechanisms; and (3) the entrenchment of a reversible cooperation structure in which health cooperation remains highly vulnerable to political and military variables. Based on these findings, this study argues that inter-Korean health cooperation should be normatively redefined not as a series of ad hoc humanitarian projects or symbolic diplomatic initiatives, but as a depoliticized safety mechanism and a core component of health security infrastructure on the Korean Peninsula, encompassing infectious disease response, protection of vulnerable populations, mitigation of regional disparities, and overall social safety stabilization. To this end, the study proposes a staged cooperation model that does not presuppose the level of North Korea’s participation, including the long-term construction of a health security foundation, cooperation frameworks linked with multilateral and international organizations, and specialized cooperation in fields based on mutual benefit. From a normative perspective, the institutionalization of inter-Korean health cooperation should not be regarded as a subordinate instrument of unification policy. Rather, it constitutes a structural necessity for maintaining social safety and public health stability across the Korean Peninsula, regardless of whether division persists or unification occurs. In this regard, the study offers significant implications for future policy design, legislative development, and the construction of governance frameworks related to inter-Korean health security cooperation.

본 연구는 남북 보건의료 협력이 지난 20여 년간 정상회담·분과위원회·실무협의를 거쳐 의제의 폭과 전문성이 확대되었음에도 불구하고, 실질적 제도화 단계로 이행하지 못한 채 반복적 중단과 재가동 을 반복해온 구조적 원인을 규명하고, 향후 지속가능한 협력체계 설계를 위한 법‧제도적 방향을 제시하는 것을 목적으로 한다. 연구 결과, 남북 보건의료 협력의 좌초는 특정 시기나 정권의 정책 실패가 아니라 ▲지원 중심의 단방향 구조가 상호책임 기반의 협력전환을 가능케 할 정책적 신뢰를 축적하지 못한 점, ▲합의의 양적 확대가 제도적 내구성과 이행지원체계로 전환되지 못한 점, ▲협력이 정치‧군사 변수 에 의해 쉽게 중단되는 가역적 구조로 고착된 점이라는 구조적 메커니즘의 상호작용에서 기인함을 확인 하였다. 이러한 분석을 토대로 본 연구는 남북 보건협력이 일회적 인도주의 사업이나 외교적 상징 프로 젝트가 아니라, 감염병‧취약계층 보호‧지역격차 완화‧사회안전 안정성을 포괄하는 비정치적 안전 메커니 즘이자 한반도 보건안보 인프라로 재정의되어야 함을 주장한다. 이를 위해 장기적 보건안보 기반 구축, 다자·국제기구 연계형 협력, 상호이익 기반의 전문분야 협력 등 ‘북한의 협력 비중을 전제하지 않는 단계형 협력 모델’을 제안한다. 남북 보건협력의 제도화는 통일정책의 하위 수단이 아니라 분단의 지속 여부와 무관하게 한반도 전체의 사회안전‧공중보건 안정성을 유지하기 위한 구조적 필요조건이라는 점에 서, 향후 정책 설계·입법·거버넌스 구축 논의에 중요한 함의를 제공한다.
