This study aimed to examine the psychological and behavioral effects of aversive sensory therapy on sex offenders with deviant sexual preferences and to explore the clinical applicability of behavioral interventions for this population. A total of 12 sessions of short-term aversive sensory therapy were conducted with sex offenders under Medical treatment and custody housed at the National Forensic Hospital in South Korea. Participants were assigned to either a treatment group or a waitlist control group. The therapeutic intervention consisted of masturbation reconditioning, masturbation satiation, and aversive stimulus conditioning, targeting a reduction in deviant sexual arousal. The findings of the study are as follows. First, demographic and psychological characteristics between the two groups were largely equivalent at baseline, confirming group homogeneity. Second, in the treatment group, statistically significant decreases were observed in sexual fantasy during masturbation and in sexual coping strategies between the pre- and post-treatment assessments. These results suggest that the therapy may effectively reduce specific sexual behaviors. Third, sexual arousal in response to deviant sexual imagery also showed a general decline, with a gradual decrease observed among participants with high baseline arousal across therapy sessions. These results indicate that aversive sensory therapy may serve as an effective behavioral approach for managing physiological sexual arousal, which is often difficult to address with traditional cognitive-behavioral methods. This study contributes to the growing body of evidence supporting the use of behavioral therapy for sex offenders in the South Korean correctional context. However, further longitudinal studies and comparative trials with other theoretical frameworks are needed to validate and generalize these findings.

이 연구는 행동치료가 일탈적 성 선호를 가진 성범죄자에게 미치는 심리 및 행동적 효과를 탐색하고, 성범죄자에 대한 행동 치료의 적용 가능성을 검토하고자 진행하였다. 이를 위해 국립법 무병원에서 정신성적장애(소아성기호증, 성도착증 등)로 인해 법 원으로부터 치료감호 처분을 받고 수감 중인 성범죄자(3호 환자) 를 대상으로 치료집단과 대기통제집단을 모집하여 총 12회기의 단기 행동치료를 실시하였다. 치료는 자위 재조건화, 자위 포만, 혐오자극 조건형성을 중심으로 구성되었으며, 참가자들의 일탈 적 성 선호 수준을 낮추는 것을 목표로 하였다. 연구 결과, 첫째, 두 집단 간 인구통계학적 및 심리적 특성의 사전 동질성이 전반 적으로 확보되었다. 둘째, 치료 집단의 사전·사후 비교 결과, 자위 중 성적 공상과 성을 이용한 대응의 수준이 통계적으로 유의 하게 감소했다. 이는 행동치료가 특정 성적 행동을 조절하는 데 기여할 수 있음을 시사한다. 셋째, 일탈적 성적 상상에 대한 성 적 흥분도 역시 전반적으로 감소하였고, 특히 초기 흥분 수준이 회기 진행에 따라 점진적으로 감소되었다. 이러한 결과는 행동치 료가 기존 개입에서 관리하기 어려운 생리적 성적 각성 수준을 조절할 수 있는 접근임을 보여준 결과이다. 이 연구는 성범죄자 대상 행동치료의 국내 적용 가능성을 탐색하였다는 의의가 있고, 향후 장기개입 연구와 타 이론을 적용한 집단과의 비교 연구를 통해 반복 검증이 이루어져야 할 필요가 있다.
