This study examined suicidal imagery and non-suicidal self-injury (NSSI) as cognitive and behavioral factors that may drive the transition from suicidal ideation to suicide attempts. We compared Korean adults with a history of suicidal ideation who had made either a single suicide attempt or multiple attempts. Data were collected through an online survey assessing suicidal imagery, NSSI, capability for suicide, suicidal ideation, and suicide attempts. Both suicidal imagery and NSSI were significantly associated with multiple attempts. Furthermore, their interactions with suicidal ideation predicted attempt frequency. When suicidal imagery and NSSI levels were high, increases in suicidal ideation were linked to higher frequencies of attempts. These findings highlight suicidal imagery and NSSI as potential mechanisms underlying repeated suicide attempts and underscore the importance of routinely assessing and addressing these factors in suicide prevention for high-risk populations.

본 연구는 자살생각을 하는 사람 중 실제 자살시도를 하는 사람의 인지행동적 요인으로서 자 살심상과 비자살적 자해의 역할을 알아보기 위한 목적에서 실시되었다. 이를 위해 평생 한 번 이상 자살생각을 경험한 한국 성인을 대상으로 단일 자살시도자와 반복 자살시도자를 비 교하였다. 연구 자료는 온라인 설문을 통해 수집되었으며, 자살심상, 비자살적 자해, 자살잠재 력, 자살생각 및 시도력을 측정하였다. 연구 결과, 자살심상과 비자살적 자해 빈도는 반복 자 살시도와 유의한 정적 관계를 나타냈다. 또한 자살심상과 자살생각, 비자살적 자해와 자살생 각의 상호작용이 자살시도 빈도에 미치는 효과가 유의하였다. 구체적으로, 자살심상 수준과 비자살적 자해 빈도가 높은 경우 자살생각 심각도가 높아질 때 자살시도 빈도가 유의하게 증 가하였다. 본 연구의 결과는 자살심상과 비자살적 자해가 반복적인 자살시도에 있어 각각 인 지적, 행동적 기제로 작용할 수 있음을 시사하며, 향후 자살 고위험군에 대한 임상적 평가와 예방적 개입에서 이들 요인을 정기적으로 모니터링하고 다룰 필요성을 강조한다.
