I examine the realities of shipboard work—prolonged isolation and severely limited access to emergency care—and show that Korea’s seafarers’ medical examination currently omits oral-health screening. I conduct a comparative analysis of ILO/IMO guidelines and national regimes (United Kingdom, Japan, the Philippines, India) and find that many leading maritime jurisdictions mandate oral examinations. I also note that peer transport sectors (aviation, rail) assess oral status because it affects fitness for duty. Given that oral disease can cause pain, sleep disturbance, and reduced concentration with safety implications, I argue that a pre-embarkation oral assessment is essential. I propose amending the Seafarers’ Act to introduce a mandatory oral examination and a self-administered medical history questionnaire, supported by standardized forms, EHR linkage, informed consent, and privacy safeguards. Although domestic statistics currently limit full quantification, I provide actionable policy recommendations and a concrete agenda for future evaluation. Contributions: I (1) identify structural gaps in the Korean system, (2) consolidate international benchmarks establishing the normative case for oral screening, and (3) present an implementable, evidence-informed reform pathway to align with global standards and strengthen shipboard safety.

선원은 장기간 고립된 해상 환경에서 근무하며, 특히 원양 항해 중에는 응급 의료기관 접근이 매우 제한적이 다. 그럼에도 불구하고, 현재 국내 선원 건강진단 제도에는 구강검진 항목이 포함되어 있지 않다. 이에 본인은 국내 제도의 구조적 한계를 규명하고, 구강검진 항목의 제도적 도입 필요성을 실증적으로 제시하고자 하였다. 국제노동기 구(ILO)와 국제해사기구(IMO)의 지침 및 영국, 일본, 필리핀, 인도 등의 건강진단 제도와의 비교 분석을 통해, 다수 의 해운 선진국에서 구강검진을 필수 항목으로 포함하고 있음을 확인하였다. 또한 항공 및 철도 등 유사 운송 직종에 서도 구강 상태가 직무수행에 미치는 영향을 고려하여 이를 평가 항목에 포함시키고 있다. 구강 질환은 통증, 수면장 애, 집중력 저하 등으로 이어져 선박 안전에 부정적 영향을 미칠 수 있으므로, 승선 전 구강 건강 평가가 필수적이다. 이에 본 연구는 국내 「선원법」개정을 통해 구강검사 및 자기기재식 병력질문표 도입을 제안하며, 이는 국제 기준 과의 정합성을 회복하고 선박 안전을 강화하는 기반이 될 것이다. 비록 국내 통계자료의 제약으로 정량적 분석은 제 한적이었으나, 구강검진 의무화의 정당성을 입증하는 데 충분한 근거를 제시하였다.
