This study was conducted to analyze and synthesize the findings of qualitative research related to the decision-making experience of healthcare providers and advance healthcare directives counselors since the implementation of the life-sustaining treatment decision system. The meta-ethnography seven-step method by Noblit and Hare was applied to analyze and integrate the qualitative studies. Six databases, including PubMed, CINAHL, RISS, and KISS, were used to search for literature exploring life-sustaining treatment decision-making experiences. A total of eight studies were included in the analysis, and three main themes emerged: institutional limitations in the decision-making process, conflicts related to communication, and points for improvement in life-sustaining treatment decision-making. This study provides an in-depth understanding of life-sustaining treatment decision-making experiences and can serve as an important resource to address difficulties arising in counseling and propose directions for improving the life-sustaining treatment decision law.

본 연구는 연명의료결정제도 시행 이후 의료인 및 사전연명의료의향서 상담사의 연명의료결정 상담 경험과 관련된 질적 연구결과를 분석하고 통합하기 위해 수행되었다. Noblit와 Hare의 meta-ethnography 7단계 방법을 적 용하여 질적 연구를 분석하고 통합하였다. 연명의료결정 상담 경험을 탐구한 문헌을 검색하기 위해 PubMed, CINAHL, RISS, KISS 등 8개의 데이터베이스를 이용하였다. 최종적으로 6편의 연구가 분석에 포함되었으며, 통합결 과 연명의료결정 과정에서의 제도적 한계, 의사소통과 관련된 갈등, 연명의료결정 보완점 등으로 3개의 주제가 도출되 었다. 본 연구는 연명의료결정 상담 경험에 대한 깊은 이해를 제공하며, 상담에서 발생하는 어려움을 해결하고 연명의 료결정법 개선 방향을 제시하는 데 중요한 자료로 활용될 수 있을 것이다.
