The purpose of this study was to identify factors associated with single and dual use of drinking and smoking in adolescents by gender. A secondary data analysis study was conducted using data from the 19th Youth Health Behavior Survey in 2023. Complex sample descriptive statistics and multinomial logistic regression analysis were performed. The single use of drinking among boys and girls was 27.8% and 22.5%, the single use of smoking was 1.5% and 0.7%, and the dual use of drinking and smoking was 9.6% and 5.3%. Factors associated with drinking and smoking single and dual use were age, school level, academic achievement, household economic status, living with family, depression, suicidal ideation, fatigue recovery from sleep, and feeling of loneliness. Economical support was only related to female students. Boys showed higher levels of single and dual use of drinking and smoking than girl and there were gender differences in associated factors. This study suggests that providing gender-specific education could be an effective strategy when developing adolescent drinking, smoking prevention education, and intervention programs, and could be used as evidence for health education and policy proposals.

본 연구는 성별에 따른 청소년의 음주, 흡연 단일 및 이중사용 관련 요인을 확인하기 위해, 2023년 제19차 청소년건강행태조사 자료를 활용한 이차자료 분석 연구이다. 자료 분석은 복합표본 기술통계, 다항 로지스틱 회귀분 석을 수행하였다. 본 연구의 결과 남, 여학생의 음주 단일사용은 27.8%, 22.5%, 흡연 단일사용은 1.5%, 0.7%, 음주, 흡연 이중 사용은 9.6%, 5.3%로 나타났다. 청소년은 음주, 흡연 이중 사용 경향이 높으며, 남학생과 여학생 모두에서 연령, 학교급, 학업성적, 가정 경제 수준, 가족 동거 여부, 우울, 자살 생각, 잠으로 인한 피로 회복, 외로움이 음주와 흡연 단일, 이중 사용의 관련 요인으로 나타났으며, 경제적 지원은 여학생만 관련 요인으로 나타났다. 남학생이 여학 생보다 음주, 흡연 단일 및 이중 사용이 높은 수준을 나타냈으며, 성별에 따른 관련 요인의 차이가 있었다. 본 연구는 청소년 음주, 흡연 예방 교육 및 중재 프로그램 개발 시 성별에 따라 맞춤화된 교육을 제공하는 것이 효과적인 전략 이 될 수 있음을 시사하며, 청소년 음주, 흡연 관련 보건교육 및 정책 제안의 기초자료로 활용될 수 있을 것이다.
