Digital Therapeutics (DTx), as software-based medical devices, are increasingly playing a significant role in disease management and treatment. This study aims to analyze the major user-related hazard that may arise for each digital therapeutics based on its context of use and propose measures to address these risks. The characteristics of the context of use and potential hazards were systematically identified for four representative digital therapeutics evaluated by Korea University Guro Hospital Usability Test Center. The study found that users' physical and cognitive characteristics, the use environment, and the type of interfaces (e.g., HMDs, controllers) significantly influenced the occurrence of major hazard. The risk control alternatives proposed in this study are expected to contribute to improving usability, enhancing patient safety, and increasing therapeutic efficacy during the design and development of digital therapeutics by manufacturers. This study deepens the understanding of hazard related to the use of digital therapeutics and emphasizes the need for further research involving diverse environments and user groups in the future.

디지털 치료기기(DTx)는 소프트웨어 기반 의료기기로서 질병 관리와 치료에서 점차 중요한 역할을 하고 있다. 본 연구는 디지털 치료기기의 사용 맥락(Context of Use)을 기반으로, 각 디지털 치료기기 별로 발생할 수 있는 주요 사용 관련 위해요인을 분석하고 이를 개선하기 위한 방안을 제시하는 것을 목표로 한다. 고려대학교 구로병원 사용적합성 시험센터에서 수행한 네 가지 대표적인 디지털 치료기기를 대상으로 사용 맥락의 특성과 잠재적 위해요인을 체계적 으로 식별하였다. 연구 결과, 디지털 치료기기 사용자의 신체적·인지적 특성과 사용 환경, 그리고 인터페이스(HMD, 컨트롤러 등)의 유형이 주요 위해요인 발생에 영향을 미침을 확인하였다. 본 연구에서 제안된 위험 통제 대안은 제조 사가 디지털 치료기기를 설계·개발하는 과정에서 사용적합성을 개선하고 환자 안전성과 치료 유효성을 높이는 데 기 여할 수 있을 것으로 기대된다. 본 연구는 디지털 치료기기 사용과 관련된 위험 요인에 대한 이해를 심화하며, 향후 다양한 환경 및 사용자 그룹을 대상으로 한 추가 연구의 필요성을 강조한다.
