The study examines the ‘Act on Integrated Support for Regional Care, including Medical service and Care,’ which is set to be fully implemented in March 2026. The research explores the historical context and legislative process behind the Act, highlights its key provisions, and addresses critical considerations necessary for the legislation to effectively fulfill its intended purpose. Additionally, the study discusses issues to be addressed when drafting and implementing subordinate legislation to ensure comprehensive policy realization. The research findings indicate that the implementation of this law, which integrates approximately 30 related regulations concerning regional care such as medical and care services, necessitates a comprehensive reorganization and revision of associated statutes through detailed examination. For the delivery system of integrated support, medical and professional judgment plays a crucial role, particularly in identifying and assessing target groups, developing individualized support plans, and establishing coordinated services. Regarding the provision of medical services, which forms the distinguishing and central aspect of this legislation, it is essential to designate regional medical institutions as primary providers. These institutions should be supported with systems that ensure efficient delivery of medical and care services and enable seamless integration with other services tailored to the unique characteristics of each local government. This law outlines the roles of the central and local governments, emphasizing the necessity of formulating strategies to support the central government’s basic plan for integrated support. Such strategies should include conducting on-site surveys, establishing and managing an integrated support information system, and addressing disparities in regional financial resources and infrastructure to prevent service gaps. Finally, beyond utilizing existing budgets, consideration should be given to allocating portions of the National Health Insurance and Elderly Care Insurance funds. To ensure the policy's long-term sustainability, the establishment of a dedicated fund for integrated care is also recommended.

본 연구는 2026년 3월 본격 시행을 앞둔 ‘의료･요양 등 지역 돌봄의 통합지원 에 관한 법률’의 제정 배경 및 경과, 주요 내용을 분석하여, 동법이 입법목적을 충실히 달성하기 위해 고려되어야 할 주요 쟁점과 하위법령 작업시 고려 사항들 을 고찰하였다. 연구결과에 의하면 이법은 의료･요양 등 지역 돌봄의 통합으로 인해 관련 법 률이 30여개에 달하는 바, 관련 내용의 면밀한 검토를 통해 법률들의 대대적인 정비와 개정작업이 선행될 필요가 있음을 확인하였다. 이법상 통합지원의 대상 자와 서비스 내용은 향후 확대의 가능성을 기반으로 그간 다양하게 진행된 정부 의 선도사업과 지방자치단체별 사업들의 대상, 돌봄서비스 내용, 제공 기관, 관 련 인력과 조직 등의 성과 평가를 바탕으로 실효성 있는 서비스 목록, 지원 체계, 조직 모형 등을 도출하고 이것이 하위 법령에 구체화될 수 있도록 준비할 필요가 있다. 통합지원의 전달체계의 경우 대상자의 발굴, 조사, 개인별 지원계획 수립, 연 계서비스 확립 등을 고려하여 의학적･전문적 판단이 중요하고, 이법의 차별성이 자 핵심 서비스인 의료서비스와 관련된 제공기관의 경우 지역내 의료기관으로 설정하여 의료･돌봄서비스를 효율적으로 제공할 수 있는 운영체계를 마련할 필 요가 있다. 또한 이법은 돌봄서비스 실행에 있어서의 중앙정부와 지방정부의 역 할을 규정하고 있는데, 지방정부는 지역별 통합지원을 실행하기 위한 실행 능력 과 통합지원 서비스 제공 체계 확립을 위한 실질적 역할을 강화하고 중앙정부의 통합지원 기본계획 수립 및 시행, 실태조사, 통합지원정보시스템 구축･운영 등의 지원을 통해 지역별 재정 및 인프라 수준에 따라 격차가 발생하지 않도록 지원 하는 방안을 강구할 필요가 있다. 더불어 통합지원 서비스의 내용은 다양한 부처와 관련이 있어 돌봄통합지원의 효율적 제공을 위한 부처별 역할과 기능을 설 정할 필요가 있다. 마지막으로 인구의 고령화에 따른 보건복지수요의 증가는 재 정의 부담으로 이어질 수 밖에 없다. 이에 기존 예산은 물론 국민건강보험, 노인 장기요양보험 재정의 일부를 고려할 수 있으며, 정책의 지속가능성을 위해 통합 돌봄을 위한 별도 기금의 설치를 제안하였다.
