Purpose: The purpose of this study is to clearly define and analyze the concept of non-compliance with treatment instructions in hemodialysis patients. Methods: Using Walker and Avant's concept analysis method, a combination of keywords such as 'hemodialysis patients', 'non-compliance with treatment instructions', 'compliance', and 'self-care' were used. The databases RISS, KISS, DBpia, and SCHOLAR were utilized to search and analyze academic journals. Results: The key attributes of non-compliance with treatment instructions in hemodialysis patients were identified as 'psychological state', 'support and motivation', 'knowledge and understanding', and 'self-efficacy'. These attributes act as antecedent factors for non-compliance, leading to outcomes such as infection, cardiovascular problems, worsening kidney function, deteriorating mental health, decreased quality of life, and reduced survival rates. Conclusion: To reduce non-compliance with treatment instructions, a multifaceted approach is necessary, including enhanced patient education, regular information provision, emotional and social support, and improvement of self-efficacy. These approaches can contribute to reducing patient uncertainty, increasing compliance with treatment instructions, and ultimately improving overall health outcomes and quality of life.

RISS, KISS, DBpia, SCHOLAR 데이터베이스를 이용하여 학술지를 검색하고 분석하였다. 결과: 혈액투석 환자의 치료지시 불이행의 주요 속성은 ‘심리적 상태’, ‘지원과 동기’, ‘지식과 이해’, ‘자기효능감’으로 나타 났다. 이러한 속성들은 치료지시 불이행의 선행 요인으로 작용하며, 결과로는 김염, 심혈관계 문제, 신장 기능 악화, 정신건강 악화, 삶의질 저하, 생존율 감소 등이 확인되었다. 결론: 치료지시 불이행을 감소시키기 위해 환자 교육 강 화, 정기적인 정보 제공, 정서적 및 사회적지지, 자기효능감 증진 등의 다각적인 접근이 필요하다. 이러한 접근은 환 자의 불확실성을 줄이고, 치료지시 이행을 높이며, 전반적인 건강 결과와 삶의 질을 향상시키는 데 기여할 수 있을 것이다.
