The purpose of this study is to examine how basic pension income mitigates the impact of medical expense overburden on depression among chronically ill older adults living alone. Previous research on the effects of the basic pension has mainly focused on economic aspects such as changes in income, expenditure, and poverty status of the older adults, overlooking non-economic aspects, in particular changes in mental health such as depression. Using data from the Korea Welfare Panel Study (Waves 10th-17th), fixed-effects panel regression analysis was conducted for the entire sample and by four subgroups: young-old men, young-old women, old-old men, and old-old women. Results indicated no significant interaction effect between medical expense overburden and basic pension income for the entire sample; however, a significant negative interaction effect was found for young-old men. The study discusses the implications of these findings for theory and policy regarding the improvement or restructuring of the basic pension system.

본 연구는 만성질환이 있는 독거 노인의 의료비 과부담이 우울에 미치는 영향에서 기초연금 소 득액의 조절효과 검토를 목적으로 한다. 기초연금 효과에 대한 선행연구들은 노인의 소득, 지출, 빈 곤지위와 같은 경제적 측면만큼 비경제적 측면, 특히 우울과 같은 정신건강의 변화에 상대적으로 관심이 적었다. 본 연구는 한국복지패널조사 자료(10차-17차)와 고정효과 패널회귀분석을 사용하여 만성질환 있는 독거 노인 전체 그리고 연령과 성별을 조합한 4개 하위집단별 분석을 실시했다. 분 석결과, 기초연금 소득액의 조절효과는 만성질환이 있는 독거 노인 중 전기남성에서만 (-)로 유의 미했다. 본 연구는 이 연구결과가 기초연금제도의 개선 또는 재구조화에 주는 이론적·정책적 함의 를 제시했다.
