Needle-free injector were developed for various reasons, including the fear of needles, possibility of proliferation of disease through reuse, unintended pricking of the needle, and possibility of infection. Although needle-free technology reduces the patients’ concerns on the use of a needle, it is not possible to fully guarantee that safety issues such as negligence by the user will not occur. The safety and applicability of the use have become an important issue due to the advancement of medical devices. As such, the appropriate use by the patient is required due to the increase in medical device accidents arising from the negligent use. Therefore, this study researched the means of improving the error in the use through the evaluation of risk-management based appropriateness with a Needle-free injector. The needle-free injector is a medical device that injects liquid by passing it through the skin by means of micro-spraying, using the magnetic force and spring. The risk is evaluated after having performed the identification of factors and harmful situations in order to identify the error in the use of the needle-free injector by applying an advisory group review technique. Based on outcome of the evaluation, a formative evaluation is executed by assessing the software and hardware user interface requirements. By applying the brainstorming technique, a scenario for the use related to harmful factors is identified. which will then be used to deduce a scenario for the use for the summative evaluation. The intended user is professional medical personnel, and the evaluation of appropriateness of the use was executed in the operating room, which is the actual environment for the use. Lastly, opinions of the developers were asked by applying FMEA (Failure Mode and Effect Analysis) to the review opinions on the outcomes of the appropriateness of the use evaluation and the outcome of the risk evaluation conducted for the details of the identification predictable for harmful factors and situations. As a result, the means of the improvement of risk-management based appropriateness of the use was deduced by reflecting the opinions of 15 intended users and 5 developers for the 17 errors by the of a needle-free injector. It is anticipated that the development of needle-free injector will be helpful in improving the safety of and appropriateness of the use by the users.

바늘 없는 주사기는 바늘 공포증과 재사용을 통한 질병 확산 가능성, 의도하지 않은 바늘 찔림 혹은 그로 인 한 감염 가능성 등을 예방하고자 개발 되었다. 바늘 없이 약물을 전달하는 기술은 바늘 사용에 대한 환자의 두려움 을 줄이는 매우 분명한 이점을 제공하지만, 기존 주사기와 다른 사용 방법으로 인해 사용자 부주의 등으로 발생될 수 있는 안전 문제가 발생될 수 있다. 이렇듯, 기술의 발달로 새로운 개념의 의료기기가 등장함으로써 제품 사용자의 안 전과 사용 편의성 등이 중요한 이슈가 되고 있다. 의료기기의 부주의한 사용으로 인한 사고가 증가함에 따라 사용자 와 환자의 안전을 위한 사용적합성이 요구되고 있다. 본 연구에서는 자기력과 스프링으로 약액을 미세하게 분사하여 피부를 통과하여 주입되는 분사식 주사기를 대상으로 위험관리 기반의 사용적합성 평가를 통한 사용오류 개선 방안 을 제안하였다. 우선, 분사식 주사기의 사용오류 식별과 위해요인 및 위해상황 식별을 수행한 후 위험을 평가하고 이 를 바탕으로 소프트웨어 및 하드웨어 사용자 인터페이스 요구사항을 파악하여 형성평가를 수행한다. 브레인스토밍 기 법을 적용하여 분사식 주사기 위해요인 사용 시나리오를 식별하고 총괄평가를 위한 사용 시나리오를 도출하였다. 의 도된 사용자는 전문 의료인으로 실제 사용 환경인 시술실에서 사용적합성 평가를 진행하였으며, 사용적합성 평가 결 과에 대한 검토 의견과 알려지거나 예측 가능한 위해요인 및 위해상황 식별 내용에 대하여 위험평가를 수행한 결과 에 FMEA(Failure Mode and Effect Analysis)를 적용하여 개발자들의 의견을 물었다. 그 결과 분사식 주사기의 17가 지의 사용 오류에서 의도된 사용자 15명과 개발자 5명의 의견을 반영하여 위험관리 기반의 사용적합성 향상을 위한 개선방안을 도출하였다. 이러한 개선사항을 반영한 분사식 주사기의 개발은 사용자의 안전성과 사용적합성 향상에 도 움이 될 것으로 기대된다.
