This study is about doctors whose track records are unknown due to a lack of data among research related to Joseon Tongsinsa. The purpose of this paper is to supplement preceding study based on Tsushima’s records. Among the members of the Joseon Tongsinsa, the positions related to medical care were ‘良医’ and ‘医員’. preceding study say that ‘良医’ was in charge of academic exchanges with the Japanese doctors, and ‘医員’ was in charge of medical treatment. Is it because of this? In many cases, records are made only with respect to ‘良医’, and ‘良医’ is marked as ‘医師’. In the ‘Tsushima’s records’, when the lord of the Tushima fell ill, the Joseon Tongsinsa’s doctor went to see him. At this time, there were cases where the General manager sent him for medical treatment, and there were cases where the Tsushima’s lord requested it himself. During the 8th Joseon Tongsinsa, on the day the Joseon Tongsinsa arrived in Huchu(府中), there was a case in which ‘良医’ and ‘医員’ were called at the request of the lord. But medical support for health care was not one-sided. Joseon Tongsinsa also requested medical support from Tsushima, but it was not limited to medicinal materials or tools. Joseon Tongsinsa mainly requested surgical supports on limited occasions.

본고는 기존 통신사 관련 연구 중에서 사료의 미비로 인해 좀처 럼 그 행적을 찾기 힘들었던 직책, 그 가운데에서도 의원에 대해 쓰시마번의 신사기록을 토대로 그 행적을 찾아 기존의 연구를 보 충하는 데에 그 목적이 있다. 통신사 수행원 가운데 의료에 관련해서는 의원과 양의의 두 종 의 직책이 파견되었다. 이중 양의는 일본측과의 교류에 중점을 두 었고, 의원은 실질적인 의료행위를 도맡았다. 이 때문인지 신사기 록 가운데 의원에 대한 기록은 거의 등장하지 않고 대개 양의에 대해서만 기록이 남아있는데, 양의는 ‘의사’라고만 지칭되었다. 신사기록에서 번주가 병환이 생기면 의원이 번주를 진료하러 갔 는데, 이는 삼사가 위문 삼아 보낸 경우도 있었으나, 번주가 요청 하는 경우도 있었다. 신묘년의 경우에는 양의와 의원이 쓰시마번 후추에 도착하자마자 번주의 요청에 따라 진맥을 보러 간 경우도 있었다. 일본 측에서 의원에게 의료행위를 요청한 케이스는 번주 뿐만 아니라, 임술사행에서 8개, 기해사행에서 4개, 무진사행에서 2개, 계미사행에서 1개를 찾을 수 있었다. 임술사행이 유독 많고 나머지는 시기를 거치면서 그 숫자가 줄어드나 시기가 뒤로 갈수 록 신분이 다양해지고 있었다. 대개의 경우 양의가 진료를 보았으 나, 간혹 의원이 진맥을 보는 경우도 있었다. 그러나 의료 원조는 일방적인 것은 아니고, 통신사 측도 쓰시마 번에 의료 지원을 요청하였다. 여기에는 약재나 도구뿐만 아니라, 직접적인 치료도 있었는데 전체적인 케이스가 많진 않지만 특히 외과적인 치료가 주를 이루었다.
