According to the Statistics Korea's forecast, Korea will be a super-aged society at the fastest pace in the world, accounting for 20.6% of the total population aged 65 or older in 2025. Long-Term Care Hospitals have increased the importance of their role as facilities that provide medical and nursing services to elderly patients and chronically ill patients. However, related regulations have been strengthened due to oversupply, prolonged hospitalization, relationship with nursing facilities, and safety issues. This study attempted to review policies related to quality management of nursing hospitals with different characteristics from other medical institutions, analyze problems, and suggest solutions to improve (related to nursing hospitals) that can ensure rationality. The current Long-Term Care Hospitals quality management policies include the nursing hospital mandatory certification system under the Medical Act, and the hospitalization benefit adequacy evaluation system under the National Health Insurance Act. The government has expanded quality management policies for increasing Long-Term Care Hospitals and patient safety. Unfortunately, the quality management policies of Long-Term Care Hospitals were not in equity with other medical institutions, and led to strengthen regulations. It is necessary to improve the mandatory certification system for Long-Term Care Hospitals switching to an auto-certification method or apply various certification standards for many kinds of Long-Term Care Hospitals. In addition, the evaluation system for the adequacy of hospitalization benefits in Long-Term Care Hospitals is also a mandatory evaluation similar to the certification system, and is a strong regulation in connection with the cost of medical care benefits. In terms of the ambiguous roles and functions of Long-Term Care Hospitals and nursing facilities, it is necessary to focus on strengthening the medical functions of Long-Term Care Hospitals and tightening the requirements for opening Long-Term Care Hospitals.

통계청 전망에 따르면 우리나라는 2025년 전체 인구 중 65세 이상 노인이 차 지하는 비율이 20.6%에 달해 세계에서 가장 빠른 속도로 초고령 사회에 진입한 다. 요양병원은 노인 환자 및 만성질환자에게 의료 및 요양서비스를 제공하는 시설로써 그 역할의 중요성이 증대되었다. 그러나 공급과잉, 입원 장기화 문제, 요양시설과의 관계, 안전사고 발생 등으로 인해 관련 규제들이 강화되었다. 본 연구는 타 의료기관과는 다른 특성을 가진 요양병원 질 관리 관련 정책들 을 검토하고 문제점을 분석하여 합리성을 담보할 수 있는 요양병원 관련 정책의 개선방안을 제시하고자 하였다. 현행 요양병원 질 관리 정책은 크게 의료법상 요양병원 의무인증제도, 인력 및 시설 등의 기준, 국민건강보험법상 입원급여 적정성 평가제도, 수가 정책 등 이 있다. 정부는 요양병원의 양적 증가, 환자 안전 관련 문제가 발생하자 질 관 리 정책들을 확대해 왔다. 이러한 결과 전반적으로 요양병원 질 관리 정책들은 타 의료기관과 형평성에 맞지 않고, 요양병원의 질 관리를 지원하는 방향이 아 닌 규제만 강화하는 정책이 주를 이룬다. 연구결과 요양병원에만 적용하는 의무인증제를 개선하여 인증제도의 목적, 취 지 등에 맞게 자율인증 방식으로 전환하거나 요양병원의 규모나 환자군 특성에 따라 다양한 인증기준을 적용할 필요가 있다. 또한 요양병원 입원급여 적정성 평가제도 역시 인증제도와 유사한 의무 평가이고, 요양급여 비용과 연계하여 강 력한 규제가 된다는 점을 확인하였다. 이에 인증제도와의 연계를 고려할 필요가 있다. 요양병원과 요양시설의 모호한 역할과 기능에 있어서는 요양병원의 의료적 기 능을 강화하고 요양병원 개설 요건을 강화하여 무분별한 양적 증가를 관리할 필요가 있다.
