The cult of the Bhaisajyaguru Buddha, also known as the Medicine Buddha, is a “this-worldly” and popular Buddhist belief that seeks to relieve disease, prolong life, and grant the wishes of believers who worship the Medicine Buddha and perform rituals related to him. This paper examines central elements and characteristics of the Medicine Buddha cult by exploring the rituals related to the Medicine Buddha. The Buddhist scriptures translated from Sanskrit into Chinese that serve as the foundation of the Medicine Buddha cult in East Asia include the twelfth volume of the apocryphal Sutra of Consecration (灌頂七萬二千神王護比丘呪經, Guanding qiwanerqian shenwang hubiqiu zhoujing or 灌頂經, Guandingjing) compiled in the fourth and fifth centuries during the China’s Northern and Southern dynasties; the Sutra on the Original Vows of the Medicine Buddha (藥 師如來本願經, Yaoshi rulai benyuan jing) translated by Dharmagupta (達摩笈多, ?~619) in 615 during the Sui Dynasty; the Sutra on the Merits of the Original Vows of the Medicine Buddha of the Lapis Lazuli Radiance (藥師琉璃光如來本 願功德經, Yaoshi liuliguang rulai benyuan gongde jing) translated by Xuanzhang (玄奘, 602~664) in 650 during the Tang Dynasty; and the Sutra on the Merits of the Original Vows of the Seven Buddhas of the Lapis Lazuli Radiance, the Masters of Medicine (藥師琉璃光七佛本願功德經, Yaoshi liuliguang qifo benyuan gongde jing) translated by Yijing (義淨, 635~713) in 707 during the Tang Dynasty. The Medicine Buddha cult stressed rituals and practices rather than convoluted concepts or doctrines. The four scriptures mentioned above describe the rituals associated with the Medicine Buddha, including offering rituals dedicated to Medicine Buddha images, the Ceremony of Enhancing Longevity with Banners and Lamps (續命幡燈法), and the Ritual of Healing Disease by Knotting Strands of Five Colors (五色縷法). These rituals served as the fundamental motifs for esoteric rites performed at an altar (or mound) for the Medicine Buddha that were included in the early Esoteric Buddhist scripture Compiled Dharani Sutra (陀羅尼集經, Tuoluonijijing), translated by Atikuta (阿地瞿多, 7th century) into Chinese in 654 during the Tang Dynasty. The Medicine Buddha ritual in the Ritual Procedure for Recitation to the Medicine Buddha (藥師如來念誦儀軌, Yaoshi benyuan niansong yigui) that was translated into Chinese by Amoghavajra (不空, 705~774) in the mid-eighth century strengthened the “this-worldly” quality of the Medicine Buddha cult as an Esoteric Buddhist ritual. To perform an offering dedicated to Medicine Buddha images, one of the three rituals described in the Sutra of the Medicine Buddha (薬師經, Yaoshijing), an image of the Medicine Buddha is enshrined and participants circumambulate the image in a clockwise direction. During the Ceremony of Enhancing Longevity with Banners and Lamps, numinous banners are hung and lamps are lighted to prolong life and promote the reincarnation of the deceased. An image of the Medicine Buddha is also enshrined in this ritual. Participants do not circumambulate the image, and instead install wheel-shaped lamps made of layers of smaller lamps in front of the image. The Ritual of Healing Disease by Knotting Strands of Five Colors is related to the Twelve Divine Generals, the supporters of the Medicine Buddha. Devotees make wishes while tying threads in five colors to the names of the Twelve Divine Generals. Once the wishes are fulfilled, the threads are untied. This practice of tying and untying the threads displays occult qualities. The absence of an image of the Medicine Buddha and the simplicity of the procedures are characteristic of this ritual. The section of the Compiled Dharani Sutra entitled “Mudra and Mantra of the Medicine Buddha of the Lapis Lazuli Radiance” (藥師琉璃光佛印呪, Yaoshi liuliguan fo yingzhou) is a combination of the three Medicine Buddha rituals discussed above. This section is imbued with esoteric overtones. The first half introduces the Ceremony of Enhancing Longevity with Banners and Lamps and the Ritual of Healing Disease by Knotting Strands of Five Colors, while the second half presents the offering ritual dedicated to the Medicine Buddha. It also adds a new regulation for enshrining an image of the Medicine Buddha as the main image at a flat, round altar, which cannot be found in the Sutra of the Medicine Buddha. The Ritual Procedure for Recitation to the Medicine Buddha compiled the existing rituals for the Medicine Buddha and adjusted their orders. According to the Ritual Procedure for Recitation to the Medicine Buddha, a flat, round altar is installed, an image of the Medicine Buddha Triad is enshrined, and individual rituals, such as the Ceremony of Enhancing Longevity with Banners and Lamps and the Ritual of Healing Disease by Knotting Strands of Five Colors, are performed. It demonstrates a higher degree of completion and is better organized than the Compiled Dharani Sutra. Some mural paintings in the Dunhuang Caves allow us to imagine the Medicine Buddha rituals held in the past. However, rather than depicting the full texts of Buddhist scriptures, these paintings show only selected or transformed themes. Thus, the ancient rituals for the Medicine Buddha are presumed to have been based on the Buddhist scriptures but performed in various manners according to the times. Since the Medicine Buddha rituals incorporated traditional shamanistic and folk elements from China, they were naturally associated with magical Esoteric Buddhism that utilized several objects during worship. In this sense, the Medicine Buddha rituals can be viewed as an interface between the Medicine Buddha cult and visual art. Among the vast body of paintings and sculptures of the Medicine Buddha in East Asia, some might have been produced for enshrinement during rituals dedicated to the Medicine Buddha. A deeper understanding of rituals for the Medicine Buddha broadens the perspective on the religious functions of the Medicine Buddha images and other forms of art related to the Medicine Buddha cult.

약사여래신앙(약사신앙)은 불교에서 동방유리광정토에 계신 약사여래를 예경하고 의례를 행하면 질병이 치유되고 수명이 연장되며 소원을 이룰 수 있다는 현세이익적인 대중신앙 체계의 하나이다. 이 논문에서는 약사신앙이 여러 기물과 행위로 표출되는 약사의례들의 내용과 구성, 흐름과 특징을 상세히 고찰해 보았다. 동아시아에서 약사신앙의 근거가 되는 漢譯 약사경은 중국 남북조시대 4~5세 기경에 성립된『灌頂七萬二千神王護比丘呪經』권12를 시작으로, 隋 615년에 達 摩笈多가 한역한『藥師如來本願經』, 唐 650년 玄奘 역 『藥師琉璃光如來本願功德 經』, 당 707년 義淨 역 『藥師琉璃光七佛本願功德經』 등이 나오면서 신앙의 확산에 기여하였다. 현세 이익을 추구하는 약사신앙은 난해한 사상이나 교리보다 의례와 실천을 중시하는 경향이 있다. 위의 4종 번역본에는 약사여래상 공양법, 續 命幡燈法, 五色縷法의 약사의례가 실려 있다. 이 의례들은 당 654년에 阿地瞿多 가 한역한 초기밀교 경전의 집대성본인『陀羅尼集經』에 수록된 약사여래 壇法의 기본 모티프가 되었다. 이후 8세기 중엽 不空金剛이 한역한 『藥師如來念誦儀軌』 의 약사의례는 본격적인 밀교의 수법으로서 약사신앙의 현세 이익적 성격을 강화 하였다. 약사경에 수록된 3종의 의례 중 약사여래상 공양법에서는 약사여래상을 香花, 幢幡, 음악과 노래로 장엄하면서 의례의 참가자들이 오른쪽으로 도는 右繞 행위의 중심에 모신다. 두 번째인 속명번등법은 續命神幡을 걸고 등불을 밝혀서 수명을 늘이거나 죽은 자를 살리는 것이 주요 목적이며, 번과 등과 불상을 함께 모시는 것이 핵심이다. 한역본마다 그 수량과 조합방식은 약간씩 다르지만, 불상은 우요예배하지 않고 수레바퀴 모양의 輪燈과 한 세트로 모시며 등불을 바치는 것이 특징이다. 세 번째인 오색루법은 약사십이신장과 관련된 수법이다. 오색실로 십이신장의 이름을 마치 인질처럼 묶고 소원을 성취하면 풀어준다는 것인데, 다소 주술적이다. 오색루법은 약사여래상이 필요하지 않고 진행방식이 비교적 간단하여 치병 등의 목적으로 동아시아에서 널리 행해졌다. 『다라니집경』권2에 수록된 藥師琉璃光佛印呪는 약사경의 3종 약사의례들을 결합시키면서 밀교 壇法의 색채를 띠고 있다. 전반부는 속명번등법과 오색루법이 함께 나오고, 후반부에서는 약사여래상 공양법을 밀교의 作壇法으로 확장하였다. 특히 약사여래상을 평면이 둥근 원단에 의례의 주존으로 안치하라는 부분은 약사 경에 없는 새로운 내용으로 주목된다. 이보다 1백년 정도 후에 한역된『약사여래 염송의궤』는 기존의 약사의례들을 종합하고 진행순서도 조정한 것이 특징이다. 먼저 둥근 원단을 마련하여 약사여래삼존상 등을 모신 다음, 속명번등법과 오색 루법같은 개별 의례를 행한다. 앞의『다라니집경』보다 의례의 짜임새 면에서 완성도가 높아졌다고 할 수 있다. 우리나라에서는 약사의례가 행해졌다는 기록이나 유물이 거의 전하지 않아서 중국 돈황석굴 벽화에 반영된 예를 통해 그 모습을 짐작할 수 있다. 이 약사여래 벽화들에는 약사의례의 주요 요소인 神幡, 輪燈, 불상이 등장한다. 그러나 그림속 7층 윤등의 위로 올라갈수록 크기가 줄어드는 형태 자체는 경전에 언급되어 있지 않고, 불상의 경우 『다라니집경』 류의 원단이 함께 그려진 예도 보이지 않는 다. 즉, 경전의 내용 그대로가 아니라 일부 요소들이 취사선택되거나 변형되며 그려졌음을 알 수 있다. 그러므로 현실의 약사의례 역시 경전을 바탕으로 하되 시대에 따라 다양한 방식으로 바뀌며 진행되었던 것으로 추정된다. 현세 이익적인 성격이 강한 약사신앙은 그 의례 또한 정통적인 불교와는 다른 중국 재래의 무속적·토속적인 요소들을 다분히 포함하고 있어서 다양한 기물을 사용하는 주술적인 밀교로 자연스럽게 이어질 만한 연결고리를 지니고 있다. 이런 면에서 약사의례는 약사신앙과 시각적인 형상이 만나는 접점이라고 할 수 있다. 동아시아에 현존하는 수많은 약사여래상 중에는 과거 여러 종류의 약사의례 에서 본존으로 모시려고 제작된 예도 있을 것이다. 이처럼 약사의례에 대한 이해는 약사여래상의 종교적 기능을 비롯하여 약사신앙과 관련된 미술을 폭넓은 관점 으로 바라보는 데 도움이 된다.
