Objectives: The study aims to provide basic data on the elderly care cost policy by analyzing the total medical cost of over 65 years of age, and analyzing the total medical cost of the elderly who exceed the upper limit of copayment. Methods: The data for the four years were generated based on the actual number of people per person for each year, and a total of 481,252 senior citizens aged 65 and 21,772 exceeded the upper limit of copayment. Results: In 2014-2015, the older the population, the higher the total cost of medical treatment, giving benefits to the older people. The significant factors influencing the total medical expenses of the elderly over 65 years were age, income group, region, serious illness, the presence of disability, total hospitalization days, total outpatient visits, and classification of disease. Conclusions: The average medical expenses of elderly who exceed their own burden limit were reduced overall after grade change of copayment, and the oldest and the low-income class was positively affected by adjustment of copayment grade.
