The Effects of Social Exclusion on Mental Health of Adult Residents in Disadvantaged Apartment Neighborhoods

This study examined the effects of social exclusion and the perception of disadvantaged neighborhoods on mental health of the residents, focusing on the depression of adults. It has long been argued that the perceived environment of disadvantaged neighborhoods may affect mental health problems of the residents. However, there has been an alternative viewpoint which emphasizes social exclusion against the poor neighborhood in affecting mental health. In this regard this study hypothesized that the perceived built environment of disadvantaged neighborhoods would indirectly affect mental health of adults through the intervening effects of social exclusion. In order to analyze the hypothesis, this study utilized the SEM model in which five subjective neighborhood variables including built environment, collective resources, sense of community, neighborhood disorder, and security from crime were employed. The model also controlled for individual characteristics such as gender, age, education, illness, place of residence and period of residency. As a diagnostic and severity measure of mental health, the Korean version of the Patient Health Questionnaire-9 (PHQ-9), originally suggested by Kroenke and Spitzer, was adopted. Data for the analysis was randomly collected from residents of inner-city neighborhoods of Daegu metropolitan city. Analysis results indicated perceived built environment was not significant in the presence of social exclusion while it was significant without the intervening variable. This study also showed sense of community in the latent variables and gender, chronic illness, and education in the control variables were significant. Based on the empirical analysis, policy implications were discussed. In particular, it was suggested that in addition to improving the physical environment of disadvantaged neighborhoods, the social inclusion policy for residents of disadvantaged neighborhoods would be indispensable for mental health.

본 연구는 도시 저소득계층이 거주하는 노후 아파트 단지의 사회적 배제가 주민들의 정신건강에 미치는 영향을 분석하는 것을 목적으로 하고 있다. 오랫동안 도시 취약지역 의 열악한 주거 환경은 우울과 같은 주민들의 정신 건강에 직접적인 영향을 미치는 것 으로 주장되어 왔다. 즉, 이 주장은 개인적 요인과는 별도로 취약지역의 열악한 물리적 환경과 사회적 환경이 거주하는 주민들의 정신건강에 영향을 미친다는 것이다. 특히 외 국의 경우 슬럼과 같은 취약지역은 건물이나 기반시설이 노후할 뿐만 아니라 범죄와 폭 력이 만연하여 주민들이 우울감을 겪는 등 정신건강이 위협을 받는 사례가 많다는 것이 실증적 연구를 통하여 보고되고 있다. 그러나 이와 같은 주장과 별도로 취약지역의 환 경적 요인보다는 이들에 대한 낙인과 사회적 배제가 주민들의 정신건강에 더 큰 영향을 미친다는 주장이 제기되고 있고 이를 뒷받침하는 실증적 연구도 있다. 그러나 우리나라 에 있어서 주민들의 정신건강에 미치는 요인과 관련하여 근린 환경에 대한 인식과 사회 적 배제의 관계를 분석한 연구는 많지 않은 실정이다. 이와 같은 측면에서 본 연구는 우 울감을 중심으로 두 변수가 노후 아파트 단지 주민들의 정신건강에 미치는 영향을 분석 하는 것을 목적으로 하고 있다. 이와 관련하여 본 연구가 설정한 가설은 노후한 취약 아 파트 주민들의 정신건강에 영향을 미치는 직접적인 요인은 사회적 배제이고 열악한 근 린 환경은 사회적 배제를 통하여 간접적으로 영향을 미친다는 것이다. 본 연구는 이와 같은 가설을 구조방정식 모형 (SEM model)을 이용하여 분석하였 다. 본 연구에서 정신건강을 설명하기 위하여 도입한 근린변수는 물리적 환경, 집합자 원, 공동체 의식, 근린 무질서, 범죄로부터의 안전성에 관한 주관적 인식이다. 그리고 통제변수로 도입된 개인적 특성변수는 그 동안의 이론적, 경험적 연구에서 유의성이 있는 것으로 밝혀진 성별, 연령, 교육정도, 질병, 거주장소, 거주기간 등이다. 그리고 종속변수인 정신건강은 우울감인데 다양한 측정도구 가운데 크뢘크와 스피츠 (Kroenke and Spitzer) 에 의하여 개발된 PHQ-9의 한글판을 측정도구로 사용하였 다. 그리고 분석을 위한 자료는 대구광역시 중구의 도심에 위치하고 있는 동 지역 주민 들을 대상으로 면접조사를 통하여 수집하였다. 이 지역에는 대구에서 가장 오래되고 노후하여 저소득계층이 거주하는 저층 아파트 단지를 비롯하여 다양한 계층이 거주하 는 주택들이 입지하고 있다. 본 연구는 실증적 분석결과를 토대로 취약 아파트 주민들의 정신건강에 영향을 미 치는 직접적인 요인은 사회적 배제이고 열악한 근린 환경은 간접적인 요인임을 밝혔 다. 그리고 본 연구는 근린 요인 가운데는 공동체 의식이 유의미한 변수이고 개인적 요 인 가운데는 성별, 만성적 질병, 교육 수준 등이 유의한 변수인 것을 밝혔다. 그리고 본 연구는 실증적 분석결과를 토대로 취약 근린 주민들의 정신건강을 유지하기 위한 근린 정책 과제로 취약한 근린의 물리적 환경을 개선하는 것이 필요하겠지만 이 근린에 대 한 사회적 포용정책이 중요하다는 것을 제시하였다.
