It was estimated that the public health expenditure is accounted for 13.4% of the total government expenditure in fiscal year 2019. According to the expenditure budget of central government, however, the public health expenditure was only 2.6% of the total expenditure. This distortion of the central government’s resource allocation is mainly due to the fact that the national health insurance account is excluded from the public finance system. Since accurate counting and execution of the budget are fundamental to performing government’s tasks, it is needed to secure a consolidated finance system of international standards and to produce budget statistics according to the classification of functions of government (COFOG). To resolve these problems inherited in the public finance management, this paper tried to estimate the total amount of government expenditure as well as the public health expenditure respectively. Second, it also tried to identify fiscal issues posed by the current decision-making structure of the health insurance. In addition, it analyzed the general aspects of foreign countries regarding the comprehensiveness of budgets related to health expenditure. Based on this analysis, this study proposes to establish a legal framework for incorporating the health insurance account into public finance system in order to enhance fiscal accountability of the national health financing.

우리나라는 정부 전체로 보면, 2019년 기준 재정총량이 693조원에 이르고 이중 13.4%인 93조원을 보건지출에 사용하는 것으로 추정된다. 그러나 중앙정부 예산회계시스템에 따르 면, 총지출은 470조원이고, 보건지출은 2.6%인 12조원에 불과하다. 정부재정 총량이 축소 되고 보건지출 규모와 배분 비중이 왜곡되는 현상은 국민건강보험 등이 국가재정 외로 운 용되며, 중앙과 지방을 아우르는 통합재정 산출기반이 취약한데 기인한다. 재정을 정확하 게 파악하고 운용하는 것은 국정의 기본이므로, 통합적인 재정관리 기반과 정부지출의 정 책기능별 배분 실상을 제대로 파악할 수 있는 제도적 틀의 구축이 우선시 된다. 국가재정 운용에 내재한 이러한 문제점을 개선하기 위하여 본 논문은 첫째, 정부 전체로 본 재정총량과 보건재정 규모를 추정하고 둘째, 건강보험 등의 재정 외 운용에 따른 문제 점을 규명하며 셋째, 주요국의 재정제도로부터 의료보장 재정관리에 관한 보편적 관점을 확보한다. 이러한 연구 결과를 토대로, 논문은 우리나라 보건재정이 책무성을 확보할 수 있는 제도적 틀을 갖추도록 건강보험 등의 국가재정 편입 방안을 제안한다.
