This study aims to analyze the characteristics of emergency medical services (EMS) utilization among elderly patients divided into two groups depending on place of living: nursing homes and places other than nursing homes. The data includes patients aged 65 years old and over for whom ambulances were requested by the 119 Dispatch Center in Gyeonggi Province from January 1st to December 31st, 2018. Statistical analysis was performed using IBM SPSS Statistics 21.0. The findings showed that EMS ambulances were requested more frequently for female patients in nursing homes than males in other places. Brain disease was the most common medical history of the EMS users at nursing homes (P=0.000). Also, critical medical conditions, such as cardiac arrest (5.4%), altered mental status (16.4%), difficulty breathing (18.6%), and fever (19.2%), were found more than twofold higher among the EMS patients in nursing homes, which were classified as emergency by EMS crews. This study suggests a review and amendment of the legal standards regarding support for emergency medical providers and allocation of ambulances.

본 연구는 노인요양시설과 그 외 장소에서 발생한 노인환자의 병원 전 응급의료서비스 이용 특성을 비교 분석하고자 2018년 1월 1일부터 12월 31일까지 경기도 119 재난종합지휘센터에 신고 되어 접수 된 65세 이상의 노인환자를 대상으로 수행하였다. 수집된 자료는 IBM SPSS statistics 21.0으로 분석 하였다. 연구 결과 그 외 장소에서 발생한 환자에 비해 노인요양시설에서 고령의 여자 환자 비율이 높았으며, 과거력 및 현병력으로 뇌질환이 많았다(P=0.000). 또한 노인요양시설에서 응급증상인 심 정지(5.4%), 의식장애(16.4%), 호흡곤란(18.6%), 고열(19.2%) 등의 발생 빈도가 높았고, 119 구급대원 이 판단하는 응급(47.4%) 환자가 많았다. 노인요양시설에서 중증의 응급 상황 발생 위험도가 높은 것으로 판단되며, 이러한 상황에 대응할 수 있도록 응급의료종사자 및 구급차 배치 등 법적 기준 검토가 필요한 것으로 보인다.
