The purpose of this case study is to investigate the meaningful result of music and imagery (MI) therapy which is one of the music therapy techniques to reduce depression of a client in a Korean oriental neuropsychiatric clinic. In an oriental neuropsychiatry clinic, six sessions of music and imagery (MI) therapy was given to one client. As a result, the client's self-report presents that music helped the client to express her feelings safely. Moreover, imagery which was induced by music helped the client to identify her own supportive resources, thereby enabling internal work such as searching for causes of fear. After MI, BDI-2 scores showed decreased level of depression of the client indicating reduced scores (27 in the pre test but 9 in the post test). Moreover, the additional Hwa-byung scale also showed a reduction of the client’s negative feeling (29 in the pre test but 18 in the post test). These results suggest that a supportive level of MI can be used as an adjunctive therapy for relieving depression of clients in Korean oriental neuropsychiatry.

본 연구의 목적은 국내 한방신경정신과 한의원에서 내원환자의 우울 감소를 위하여 음악치료 기법 중 하나인 음악과 심상(Music and Imagery: MI) 치료를 실시하였을 때의 의미 있는 결과를 보기 위한 사례연구이다. 연구 절차 및 방법으로는 한방신경정신과 한의원에 내원하여 주요우울장애로 진단된 내담자 1인에게 6회기의 지지적 수준의 음악과 심상(MI)치료를 실시하였으며 내담자의 자기보고 및 사전 사후 BDI-2를 실시하였다. 그 결과, MI 치료는 매 회기 음악이 내담자 감정의 안전한 표현을 도왔고, 음악으로 유도된 심상은 내담자의 지지적 자원을 다시 확인하는 데 도움을 주어 두려움 원인 탐색 등의 내적 작업을 가능하게 하였음을 나타내었다. 또한 BDI-2 점수는 MI 치료 후 내담자의 우울개선(BDI-2 사전 27, 사후 8)을 나타내었다. 더불어 추가적으로 실시한 Hwa-byung척도에서 내담자의 화가 감소(사전 29, 사후 18)되었음이 나타났다. 이러한 결과는 지지적 수준의 MI 치료가 한방신경정신과 한의원 내원환자의 우울완화를 위하여 부가적 치료로 사용할 수 있는 가능성을 시사한다. 나아가 효과적인 치료를 위해서는 생의학적 접근 외에 통합적(integrative)이고 전체적(holistic) 접근의 중요성 및 기존 생의학적 치료에서의 예술치료 도입의 의의를 시사한다.
