The primary factors for successful aging would be to maintain higher physical and cognitive functioning across the life course while minimizing health risks and disability. It could make old adults maintain independence until the end of their life. Ways to successful aging might be dependent upon choosing the healthy life styles rather than inherited factors. The phenomena of declining or dropping in cognitive performance at the end of life would be found from many old adults, and it is predictive of mortality risk. The phenomena have several meanings that they might have suffered from at least a couple of chronic diseases and might not concern about health for the second half of their life while they were young. The concept of the terminal cognitive decline was discussed in the context of the successful aging and its association with chronic diseases in this paper.

성공적 노화의 요건은 최소한 질병과 장애가 없는 신체 기 능 및 인지기능을 높게 유지하는 것이다. 그와 같은 요건을 잘 갖 출 경우 생의 후반기까지 독립성을 유지하는 삶이 가능하다. 성공 적 노화에 이르는 삶은 선천적이 아니라 생활양식의 선택 등 후천 적 요인이 좌우한다. 생의 후반기에 인지기능이 심하게 저하되는 현 상(terminal decline or drop)은 몇 년 후 사망하게 됨을 예측해주는 데, 이러한 현상은 상당수의 노인들에게서 나타난다. 이는 그동안 여 러 가지 신체적 질환 등으로 인해 인지기능 저하도 초래했음을 의 미함과 동시에 젊은 시절부터 노후를 위한 건강관리 실천을 제대로 하지 못하고 살아왔음을 의미한다. 본고에서는 생의 종말기(terminal stage)에 나타난 인지기능 저하와 사망위험의 예측, 그리고 각종 질 환하고의 관련성 등을 정리하면서 성공적 노화의 방향을 논했다.
