Over the past decade in Korea, discussions on advance directives (ADs) have increased. This paper was to systematically review the concept and current state of ADs in South Korea, Japan, the USA, and the Netherlands. ADs are one method proposed to facilitate the process of communication among patients, families, and health care providers regarding the plan of care of a patient who has lost the ability to communicate. ADs might be useful in the Korean setting when an individual wishes to either 1) not provide a lot of leeway to surrogates and/or caregivers, and/or 2) ensure his or her directives in the cases of terminal illness, brain death, pain treatment, and information disclosure. Health care provider roles are vague in terms of responsibility for ADs discussion and education in Korea. Further exploration of the attitudes, knowledge, and practices concerning ADs of nurses and primary health care providers is recommended to provide focal points for future research in order to facilitate peace of mind at end-of-life for patients and families.

사전의료의향서(AD)는 대상자가 연명치료가 아니면 생을 이 어갈 수 없으며, 스스로 의사결정을 할 수 없는 상태일 때 자신의 임종기에 이루어지는 의료행위에 대한 본인의 의사를 미리 밝히는 문서를 의미한다. 또한 의사소통능력을 상실한 대상자의 케어계획과 관련하여 대상자, 가족, 건강제공자 사이의 의사소통과정의 촉진을 위해 제공되는 하나의 방법이라고 볼 수 있다. 본 연구는 한국, 일 본, 미국 및 네덜란드를 중심으로 AD 관련 법률 및 현황 등을 체계 적으로 고찰했는데, 이는 우리나라에서 시행될 수 있는 AD 정책수 립과 운영 방안 등에 대한 기초자료를 제공하기 위해서였다. AD는 1) 대상자가 건강제공자에게 케어관련 재량권을 넘겨주기를 원치 않 거나 2) 말기 질환, 뇌사, 통증관리, 정보 공개의 경우에 대상자의 케어관련 의향을 보장하기를 희망할 때 시행하는 것이 유용하다. 우 리나라 실정에 맞는 AD 작성을 위한 체계를 구축하기 위해서는 건 강할 때 대상자가 사전의료계획을 논의하도록 지원하며, 대상자와 가족이 삶의 마지막 계획을 미리 준비하는 기회를 충분히 가질 수 있도록 AD 관련 제도를 하루빨리 정립시킬 필요가 있다.
