Feeding and swallowing problem in children means not only swallowing problems, every issues associated with swallowing, feeding, and eating in their development. Especially, multiple feeding therapy is needed including Sucking-swallowing-breathing coordination in NICU, eating various type of food, using utensils, eating with social interaction, etc. Medical, physical, cognitive, behavioral, psychologic, social issues are causes that affect feeding and swallowing, and interaction with parent and children is important. To parent, feeding their child could be new occupation in one part of caring. As feeding specialist, Occupational therapist should assess, treat in various way, and understanding of parent-child feeding interactions and parent education is essential in treatment. For qualitative treatment service, therapist must get education of basic knowledge, keep attending workshop or seminar, have various experience, and get supervison and mentoring could be considered. So, occupational therapists in pediatric dysphagia should try their best continuously to give qualified treatment by understanding and experience about assessment and treatment of feeding development, feeding characters, parent-child feeding experience.

아동에서의 연하 장애는 삼킴의 문제 뿐 아니라, 발달 상에 일어나는 먹기와 관련된 모든 문제를 말한다. 특히 아동은 신생아 중환자실에서의 빨기 패턴부터 다양한 음식 먹기, 도구 사용하기, 사회적 상호작용 하여 먹기 등 성장과 발달에 따라 문제가 발생하는 원인이 다양하여 다각적인 연하 재활 치료 접근이 필요하다. 아동의 먹기에 영향을 주는 요소는 의학적, 신체적, 인지적, 행동적, 심리적, 사회적 요인 등이 복합적으로 작용하게 되며, 특히 보호자와의 상호작용 또한 중요하다. 보호자에게 있어 먹이기는 양육의 한 부분으로 새로운 작업이 되기 때문에 작업 치료사가 관심을 가져야 할 부분이다. 연하 재활 치료 전문가로서 작업치료사는 연하 문제를 다각적으로 평가하고 먹이기 기술 등에 대한 이해를 통한 다양한 치 료법을 사용해야 하며, 보호자와 아이와의 상호작용에 대한 이해 및 보호자 교육 역시 필수적으로 활용해야 한다. 높은 수준의 연하 재활 치료 제공을 위해, 치료사는 기본적인 교육 이수, 지속적인 교육 참여, 다양한 경험, 가능한 경우 심 도 있는 기술을 가진 치료사에게 슈퍼비전 및 멘토링을 받는 것도 고려할 수 있다. 따라서 아동 연하 재활 분야의 작업치료사들은 아동의 먹기 발달, 먹기 특성, 보호자와의 상호 관계, 원인에 대한 복합적인 분석 및 치료 접근에 대한 이해와 경험을 통한 질 높은 치료 서비스를 제공하기 위해 지속적인 노력을 기울여야 할 것이다.
