Extracorporeal shockwave lithotripsy system(ESWL), a medical device used for urinary and kidney stone treatment, has witnessed growth of the market as the number of stone patients has recently increased worldwide due to change of diet. Contrary to the expansion of the demand, however, test methods for evaluating performance of the system has not been established yet. Since performance of extracorporeal shockwave lithotripsy system is directly related to that of shockwave, it is necessary to develop methods of evaluating performance of shockwave. However, it has been revealed that there is no country that provides specific guidelines to test shockwave performance in the world. Thus, this research developed a guideline for evaluating safety and performance of extracorporeal shockwave lithotripsy system. The study investigated measures to evaluate performance of shockwave through literature review on the principle of extracorporeal shockwave lithotripsy system & mechanism of generating shockwave, relevant standards and approval status at home and abroad, etc. The guideline, the outcome of this research, presents explanation and detailed test methods for additional items of IEC 60601-2-36:2014, the latest international standard of extracorporeal shockwave lithotripsy system for safety test. For performance test, the test items and suggested test methods were developed by referring to Korean approval documents of ESWL, FDA guidance and IEC 61846:1998. Moreover, the final guideline has been completed through modification and supplementation process by a consultative group including experts from manufacturing and importing companies, authorized testing institutes and academia. This study aims to suggest a guideline for evaluating safety and performance of extracorporeal shockwave lithotripsy system in accordance with the growing technology.

체외충격파쇄석기는 요로 및 신장 결석 치료에 사용하는 의료기기로서, 최근 식습관의 변화로 인해 결석 환자 가 증가함에 따라 세계적으로 체외충격파쇄석기 시장은 증가하고 있다. 이와 같이 해당 의료기기에 대한 수요가 증 가하는데 비해 체외충격파쇄석기에 대한 성능을 입증하기 위한 평가방법이 확립되어있지 않은 실정이다. 충격파에 대 한 성능은 체외충격파쇄석기의 성능과 직결되므로, 충격파의 성능을 입증하기 위한 방법이 확립되어야하나, 전 세계 적으로 충격파 성능을 평가하기 위한 상세한 방법을 가이드라인으로제시하고 있는 국가는 없는 것으로 조사되었다. 그리하여 본 연구에서는 체외충격파쇄석기에 대한 안전성 및 성능을 평가하기 위한 가이드라인을 마련하였다. 본 연 구는 체외충격파쇄석기의 원리, 충격파 발생 원리, 국내·외 관련 규격, 국내·외 허가현황 및 관련 문헌 조사 등을 통한 충격파 성능 시험방법에 대한 조사를 수행하였다. 본 연구의 결과물인 가이드라인에서는 안전성 시험은 체외충 격파쇄석기 관련 최신 국제규격인IEC 60601-2-36:2014에 추가된 항목에 대한 해설과 상세한 시험방법을 제시하였으 며, 성능 시험은 국내 허가 현황, FDA 가이던스, 국제규격 IEC 61846을 참고하여 시험항목 도출 및 시험방법을 제 시하였다. 또한, 제조·수입 업체, 공인시험검사기관, 학계 등을 포함한 전문가협의체를 통해 도출된 가이드라인에 대 한 검토를 받아 수정보완 과정을 거쳐 최종 가이드라인을 확립하였다. 따라서 본 연구에서는 성장하는 기술 발전에 맞춰 체외충격파쇄석기의 안전성 및 성능을 평가 할 수 있는 가이드라인을 제안하고자 한다.
