The study analyzes if private insurance increases the demand of health services using the Korea Health panel. Korean private insurance has both supplementary and complementray features, providing both supply side and demand side incentives for increasing health service utilization. The paper estimates the effect of private health insurance on in-patient and out-patient service utilization, and on the health expenditures. Using the two-stage random effect panel IV estimation, the paper deals with endogeneity issues associated with adverse selection of private health insurance, isolating moral hazard of health insurance from adverse selection. The estimation result shows that private health insurance increases both in-patient and out-patient health utilization, and health expenditures as well. It implies that Korean private health insurance does increase national health expenditure by increasing national health insurance expenditure as well, and may need to be amended to enhance efficiency of national health system.

실손형과 정액형 민간의료보험 가입 여부가 의료서비스 이용과 의료비에 미치는 효과를 분석하였다. 실손형 보험과 정액형 보험은 외래서비스 이용과 입원횟수, 외래 의료비 및 전체 의료비를 증가시키는 것으로 나타났다. 민간보험으로 인한 과다 의료서비스 이용의 유인을 없앨 수 있도록 향후 민간의료보험, 특히 실손형 의료보험에 대한 정책방향을 새로이 정립할 필요가 있다. 건강보험 보장성 강화정책이 시행되고 있는 시점에서 민간의료보험이 불필요한 국민의료비 증가를 유도하지 않도록 하기 위해서는, 공적의료보험이 보장하지 못하는 비급여 항목을 보충하는 보충형 위주로 설계될 필요도 있다. 그러한 측면에서는 공적의료보험의 본인부담금을 일부 보장하고 있는 실손형 민간의료보험의 확대는 신중하게 이루어져야 할 필요도 있다. 특히 경증질환에 대한 보완형 민간의료보험에 대한 제도개선은 필요한 부분이다.
