A Study on the Long-Term Care Insurance System for the Elderly with Focus on Germany, Japan, and Korea

Since the middle of the 20th century, the greatest change in most countries of the world is the intensification of the aging phenomenon, resulting from the increase in aging population. Korea is no exception. During the years 1999 and 2000, Korea entered the aging society as the aging population accounted for 7% of the entire population, and its aging has proceeded fastest in the world. Such an aging society has brought about the issue of supporting the senior citizens together with the increase in medical expenses for the aged, and the issue that has expanded from family problems to social problems that need the government intervention. From the middle of the 1970s, Germany recognized the independent need for long-term care, and underwent active discussion about it and the Pflegeversicherung bill was passed in the Reichstag on April 22, 1994, the 11 volumes of German social law books were compiled, and payments of facilities salaries were enforced effective July 1996, and Japan also has enforced the Long-term Care Act effective April 1, 2000. Due to this, Japan abolished the system of preferential measures for the low-income class which had been the basis of social welfare and restructured the social welfare system into the insurance system with emphasis given to the middle class. Korea, which entered the aging society slightly later than this, began to discuss countermeasures by establishing so-called the Long-term Care Insurance System for the Elderly Planning Group’ within the Ministry of Health and Welfare, and ‘the Long-term Care Insurance System for the Elderly Law’ was enacted in the 226th provisional session of the National Assembly on April 2, 2007 (Law No.8493, promulgated on April 27, 2007), and the long-term care insurance for the elderly was enforced on July 1, 2008. So this study examined the long-term care insurance system for the elderly that has been enforced for 2 years and derived expected problems by conducting comparative analysis on the systems of advanced countries that have already enforced similar systems, particularly, the above-mentioned systems of Germany and Japan, and then attempted to present six practical policy implications that can contribute to maintaining sustainable system and successful establishment of the system. First, the solutions to the problems of regional imbalance of long-term care organizations (facilities) of Korea should be explored. Second, the expenses that the users bear should be gradually reduced. Third, the schemes of vitalizing to secure specialized manpower should be materialized. Fourth, the schemes of enhancing the quality of the long-term care services should be presented. Fifth, the schemes of vitalizing the recipients of pay should be explored. Sixth, concrete ways of securing financial stability should be explored. From now on, all the countries of the world will turn into a new world that has never been experienced before. It is a super-aged society following the aging phenomenon. This is certainly a crisis and a challenge. It should be remembered that wise choice and thorough preparations only can overcome the crisis and make new opportunities.

20세기 중반 이후, 세계 대부분의 국가들의 변화 중 가장 큰 변화는 노인인구의 증가에 따른 고령화현상의 심화라 할 수 있다. 한국도 예외는 아니다. 1999년에서 2000년을 접어들면서 전체 인구 중 노인인구 비율이 7%대를 넘어서면서 고령화 사회에 진입하게 되었으며, 세계에서 가장 빠른 속도로 고 령화가 진행되고 있다. 이러한 고령화 사회는 노인의 의료비 증가와 더불어 노인 부양문제를 초래하게 되어 한 가정의 문제를 넘어, 국가의 개입이 필요한 사회문제로 확대되고 있다. 독일은 이미 1970년대 중반부터 독자적인 장기요양의 필요성이 인식되면서 20여 년 간 활발한 논의를 거친 후, 1994년 4월 22일에 수발보험(Pflegeversicherung)에 관한 법이 의회에서 통과됨으로써 독일사 회법전 제 11권이 편제되었고, 1996년 7월부터 시설급여를 실시하였고 일본 역시, 개호보험법(介護保險 法)을 2000년 4월 1일부터 시행하고 있다. 일본은 이로 인해 그동안 사회복지의 근간이 되어온 저소득층 우선의 ‘조치제도’가 해체되었고, 중산층 중심의 보험제도로 사회보장제도를 재편성하게 되었다. 이에 비 해 조금 늦게 고령화 사회에 접어든 한국은 2000년 보건복지부내 소위 ‘노인장기요양보험 정책기획단’을 발족하여 그 대책을 논의하기 시작하였으며, 제226회 임시국회에서 ‘노인장기요양보험법’으로 2007년 4월 2 일(법률 제8493호, 2007년 4월 27일) 공포되어 장기요양보험제도가 2008년 7월 1일부터 시행되고 있다. 이에 본 연구에서는 만 2년을 지나고 있는 한국의 노인장기요양보험제도를 고찰해 보고, 이와 유사한 제도를 이미 시행하고 있는 선진국들 즉, 위에서 소개한 독일과 일본의 제도와 비교 분석하여 봄으로서 예상되는 문제점을 도출하고, 한국에 주는 시사점을 모색함으로써 지속적인 제도의 유지와 더불어 성공 적인 정착에 이바지할 수 있는 정책․실천적인 함의 여섯 가지를 제시하고자 한다. 첫째, 한국의 노인장 기요양기관(시설)의 지역별 불균형문제에 대한 해결 방안을 모색하여야 한다. 둘째, 이용자의 본인부담 률을 줄여나가야 한다. 셋째, 전문 인력 확보를 위한 활성화 방안을 구체화시켜 나가야 한다. 넷째, 노인 장 기요양 서비스 질제고 방안이 제시되어야 한다. 다섯째, 급여대상자의 활성화 방안을 모색해 나가야 한다. 여섯째, 재정적 안정을 도모하기 위한 구체적인 방안을 모색해 나가야 한다. 앞으로 세계의 모든 국가는 한 번도 겪어보지 못한 새로운 세계로 접어들게 될 것이다. 바로 고령화 현상에 따른 초 고령사 회가 그것일 것이다. 이는 명백한 위기이며, 도전이다. 앞으로 현명한 선택과 철저한 준비만이 위기를 극 복하고 새로운 기회를 만들 수 있다는 것을 명심해야 할 것이다.
