The purpose of this study is to analyze the logic and process of implementation of the German irrigation insurance reform through the Enhancement Law. An ideal type is presented as a starting point for analyzing the contents of care insurance for long - term care. The usefulness of social care is based on 'universal coverage' and 'adequacy of benefit level'. The analysis concept is 'de-commodification of cost' and 'de-familyization of care'. Germany 's care insurance based on Social Code XI is designed to provide a systematic guarantee of social consensus on the need for long - term care, To expand and strengthen care services. In particular, from 2017, five new care classes and rating tools have been introduced to seek structural changes in care insurance. In conclusion, we present the reform agenda of long - term care for the elderly in Korea through experience in Germany.

본 연구는 독일 수발보험 개혁의 이행논리와 과정을 수발강화법을 통해 분석하는 문헌연구이다. 장기요양을 위한 수발보험의 내용분석을 위한 출발점으로 이념형적 모델이 제시된다. 사회적 수발의 유용성은 ‘보호범위의 보편성’(universal coverage)과 급여수준의 충분성'(adequacy of benefit level)에 근거하고 있다. 이에 대한 분석개념이 바로 ‘비용의 탈상품화’와 ‘돌봄의 탈가족화’이다. 사회법전(SGB) XI에 근거한 독일의 수발보험은 장기요양보호의 필요성에 대한 사회적 합의를 제도적으로 보장하기 위해 2002년 수발서비스확충법, 2013년 수발혁신법 그리고 2015년부터 연차적으로 수발강화법을 제정하여 수발서비스를 확대 및 강화하고 있다. 특히 2017년부터는 5개의 새로운 수발등급과 등급판정 도구를 도입하여 수발보험의 구조적 변화를 모색하고 있다. 결론에서는 독일의 경험을 통해 한국 노인장기요양의 개혁과제를 제시한다.
