The purpose of this study is to analyse the impact of accessibility to medical facilities and public transportation on perceived health of urban and rural elderly. Since the influence from accessibility to medical facilities and public transportation on perceived health of elderly depends on their health status, this study applies generalized ordered logit model which relaxes parallel lines assumption. The results show enhanced accessibility to medical facilities lowers the possibility that the perceived health status of rural elderly is bad or very bad, and better accessibility to public transportation reduces the likelihood of rural elderly having lower perceived health status. In particular, the influence of accessibility to medical facilities and public transportation on perceived health is greater among the elderly in worse health condition. On the other hand, the perceived health of urban elderly is not affected neither by the accessibility to medical service nor by public transportation accessibility. It implies that weak accessibility to medical facilities and public transportation impedes utilization of medical service in rural area, and the elderly in lower health status is more vulnerable to the lack of access to medical service.

본 연구는 의료시설 및 대중교통 접근성이 농촌 및 도시 거주 노인의 주관적 건강상태에 미치는 영향을 분석하였다. 특히 건강상태에 따라 의료시설 및 대중교통 접근성 등에 의한 영향력의 크기가 상이하므로 종속변수의 모든 범주에서 설명변수의 영향력이 동일하다는 평행선 가정을 완화하여 일반화된 순서형 로짓모형을 적용하였다. 의료시설 접근성과 대중교통 접근성이 양호한 경우 농촌 노인의 주관적 건강 상태가 나쁠 확률이 감소하나 도시 노인의 건강상태에 대한 영향은 통계적으로 유의하지 않았다. 특히 의료시설 및 대중교통 접근성과 농촌 노인의 건강상태 간의 긍정적 상관관계는 주관적 건강상태가 보통 이하인 경우에 보다 뚜렷하게 나타났다. 이는 차량 보유율이 낮고, 의료 시설과의 거리 및 이동시간이 긴 농촌지역에서 의료 서비스 이용에 따르는 제약이 크며, 이러한 환경적 요인이 주관적 건강상태에 부정적으로 작용할 수 있음을 시사한다. 농촌지역의 건강 취약 노인 계층의 의료 서비스 이용 제약 완화를 위해 대중교통체계의 효율적 개선, 이동식 의료 서비스의 확장 등의 정책적 접근이 필요할 것이다.
