This study aims to analyze rate of implementation of basic and paramedic emergency care and the aspects of emergency care by EMT-Paramedics. For the study methods for this purpose, comparison groups for rural and urban models of each centers were selected to analyze the administration of the paramedics emergency care. And opinions of EMT-Paramedics on paramedics emergency care were collected through a survey and proposed obstacles and items for improvements in the paramedic emergency care. The statistical analysis on emergency activity by selecting rural and urban model showed that the rate of administration for basic emergency care was high in both models, whereas paramedic emergency care was very poor both models. According to the survey analysis of emergency medical technicians, they proposed that when transporting the patients within the short distance of 20 minutes of EMS activity time, quick transport to the hospital is required after basic emergency care. In the next study, our purpose was to analyze the obstacles to the paramedics emergency care and propose solutions for improvement through analysis of survey. First of all, many technicians appealed about the shortage of emergency technicians who can perform emergency care in the ambulance vehicle for two people including the driver and the legal limitations of the EMT-Paramedic's sphere of work and administration of basic emergency care. To improve the current situation, proposals have been made in the order of : placement of three qualified person(s) per ambulance vehicle, legal reformation of EMT-Paramedics' sphere of work, improvement of emergency care performance through participation in professional education, necessity of indirect medical control based on service guidance and stimulation of direct medical control and improvement in the awareness on emergency care for emergency medical technicians.

1급 응급구조사에게는 사고현장에 출동하여 병원으로 이송하는 단계에서 심정지 환자를 비롯한 응급환자에 대해 높은 전문응급처치가 요구됨에도 불구하고 구급대원의 인력부족, 마인드 부족, 업무범위가 가진 법적 한계성 등의 저해요인으로 인해 낮은 시행률을 보이고 있다. 이러한 문제점을 개선하기 위하여 본 연구는 구급활동의 거리와 시간에 따른 기본 및 전문 응급처치 시행률과 1급 응급구조사의 응급처치 현황을 살펴봄으로써 현장에서의 전문 응급처치 저해요인을 파악하고 개선방안을 제시하고자 하였다. 본 연구에서는 농촌과 도시 각 일개 센터를 대조군으로 선정하여 기본 및 전문 응급처치 시행률을 조사하였으며, 1급 응급구조사를 대상으로 설문조사를 실시하여 전문응급처치 시행에 대한 의견을 조사하였다. 조사 결과 농촌형과 도시형 모두 기본 응급처치 시행률은 높은 반면 전문 응급처치 실시는 매우 낮은 것으로 나타났으며, 구급활동 거리와 시간에 따른 전문 응급처치 요구는 높았으나 구급대원의 인력부족과 1급 응급구조사의 업무범위 법적 한계성 등이 드러났다. 본 연구는 이러한 결과를 바탕으로 향후 전문 응급처치를 개선하기 위해 구급차 1대에 유자격자 3인 탑승, 1급 응급구조사의 업무 범위에 대한 법률적 개선, 전문화된 교육 참여를 통한 응급처치 능력 향상, 업무지침에 의한 간접 의료지도의 필요성 및 직접 의료지도의 활성화 등의 정책적 제언을 제시하였다.
