This paper aims to improve the support system of using LTC services, reviewing characteristics or components of LTC insurance system and results of administrative statistics. Firstly, there are several issues in LTC insurance such as, for example, disagreement between the insured and beneficiaries relationship, deficiency of administrative continuity between insurer and local government, restrictions of benefit level and so on. Secondly, it is said that increase of beneficiaries more than it was expected, increase of visiting-service quantities, existence of not-service users, over-supply of some service infras and so on occurred. In conclusion, it is necessary that the construction of compulsory LTC service support system be established to use appropriately, controlling the amount of unneeded services in order to stabilize insurance finances in the long run. To do so, at first care management system should be constructed immediately, targeting beneficiaries free of service charge and in the future it is needed to be develop the model of the integrated care management system centered around a community.

노인장기요양보험제도의 도입으로 장기요양보호가 필요한 빈곤‧저소득층 이외의 일반계층도 가족의 간병수발의 고통에서 벗어날 수 있는 계기가 마련된 것으로 생각한다. 그러나, 동제도가 장기적으로 안정적인 상태에서 운영되기 위해서는 필요이상의 서비스가 제공되어서는 안되기 때문에 철저하게 도덕적 해이가 발생할 수 있는 여지를 파악하여 대책을 수립할 필요가 있다. 본 연구는 현행제도가 지니고 있는 특징적인 요소를 파악하고, 제도 도입후 나타나고 있는 일부 통계적 분석을 통하여 제도의 개선내용을 정리하는데 그 목적을 두고있다. 제도적 특징으로는 가입자와 수급자간 불일치, 지자체와의 연계성이 부족한 보험자운영체계, 보험급여방식의 혼용 및 급여수준의 제한, 보험료의 인상 등을 지적하고 있고, 제도시행결과를 보면 수급대상자의 확대, 방문형서비스 및 복지용구중심의 급여지출의 확대, 서비스 미이용의 발생, 서비스지불단가의 인상, 서비스인프라의 일부과잉 등이 나타나고 있다. 특히 보험재정의 안정을 위해서 필요이상의 서비스이용을 억제하여 적절하게 유도할 수 있는 서비스이용지원체계의 구축이 필요한 것으로 나타나고 있는데, 이를 위해서는 어떠한 형식이든지 간에 우선적으로 법정의 서비스 이용부담이 없는 대상자를 중심으로 한 케어매니지먼트체계의 구축이 시급하게 필요하고, 장기적으로는 지역사회중심의 케어매니지먼트체계의 구축으로 발전시킬 수 있는 모형개발이 요구된다.
