The Role of International Organizations for the International Harmonization of Pharmacovigilance

In drug assessment, the issue of safety is generally regarded as a scientific concern and the socio-cultural aspects of drug safety are often ignored, which is unfounded. Phenylpropanolamine (PPA), an agent that is believed to be associated with an increased risk of a hemorrhagic stroke, was previously withdrawn from the market by the United States Food and Drug Administration (FDA), although the drug is still in use in Europe and Japan. In 2004, after a year-long epidemiological study found the lack of a clear correlation between use of the drug and the incidence of stroke, the Korea FDA (KFDA) nonetheless decided that PPA should be withdrawn from the market because of public apprehension and the existence of a viable substitute, pseudoephedrine. Despite the action of the KFDA, which was regarded as too harsh toward the domestic pharmaceutical industry at that time, the Korean public was not satisfied with the measure and worried about the fact that the agent had been used for the last 40 years. This isolated incident leads to a fundamental question. Why was the drug prohibited for use in the United States while still being marketed in Europe? If drug safety were determined solely by the scientific facts, only a single conclusion would be possible. Although there may be racial, genetic and other differences among countries, if drug safety were entirely a scientific issue, the interpretation of the same scientific data and conclusions in the decision-making process on restrictions in the use and availability of drugs would not differ among countries, but instead would be dictated solely by science. Considering the fact that a completely safe drug does not exist, it is necessary for the world, including international organizations for pharmacovigilance, to cooperate to develop “decisionmaking algorithms,” to bring about the comprehensive consideration of all the factors and the development of consistent conclusions on the issue of drug safety. Such decision-making algorithms would also make it possible, on the international level, to minimize exposure to drug-related risks and reduce the time and cost required for the assessment and management of drug safety. Consequently, such algorithms would contribute to the promotion of international harmonization of pharmacovigilance and thereby to human health and welfare. This paper explores the social -cultural aspects of drug safety, pharmacovigilance-relevant international organizations, the lessons learned from a specific incident involving phenylpropanolamine (PPA) that occurred in Korea, and the future direction of international harmonization in the field of pharmacovigilance.

약물은 잘못 사용하였을 경우 또는 지시된 대로 사용되었을 경우에도 가끔 예상하거나 또는 예상하지 못한 이상반응 등을 나타낸 다. 그런 의미에서 약물이상반응 또는 약물과 관련된 문제점을 찾아내고, 평가하고, 이해하고 예방하기 위한 과학인 “약물감시 (Pharmacovigilance)”가 중요하다는 사실을 세계는 점점 더 깨닫고 있다. 약물의 안전성을 확보하기 위하여“약물감시”는 1) 안전성 관 련 정보의 수집, 2) 정보의 평가, 그리고 3) 평가 결과 얻어진 행정적 조치사항을 보건전문가 및 일반인에게 전파하는 3단계 과정을 거 치게 되며, 여러 국제 기구들이 설립되어“약물감시”의 단계별로 어떠한 방법을 사용하는 것이 효율적일 것인지를 논의하고 해마다 더 욱 활발한 활동을 하고 있다. 그러나 아직은 여러 면에서 이들 기구의 활동은 갈 길이 먼 것이 아닌가 생각된다. 우선 나라별로 약물 의 부작용에 대한 보건전문인이나 일반인의 보고가 활발하지 못하다. 부작용을 신고하는 데 따르는 번거로움, 경우에 따라서는 경제 적∙시간적 불이익을 받을 수 있다는 우려, 또는 평가하기에 적절한 정보를 담아 보고하는 데 필요한 지식이 부족한 점 등이 보고가 적은 주요 원인일 것이다. 이미 보고서 양식에 대해서는 국제적인 조화가 시도되고 있지만 이러한 문제점들을 해결할 수 있는 완벽한, 그리고 국제적인 통용에 문제가 없는 수준에는 이르지 못하고 있다. 두 번째로 안전성 평가에 있어서 특별히 고려되어야 할 것은 약물 의 안전성이라고 하는 용어가 일견 과학적 용어로 보이지만 실상은 사회문화적인 인자에 의해 상당한 영향을 받는 용어라는 사실이다. 예컨대, 미국 FDA가 2000년 출혈성뇌졸중 위험 문제로 미국 내 사용을 금지한 phenylpropanolamine (PPA)에 대하여 우리나라를 비롯 하여 사용 금지 결정을 내린 나라도 있고, 아무런 조치도 취하지 않은 나라도 있으며, 영국과 같이 그 사용을 제한은 하되 금지하지는 않은 나라도 있는 등 나라마다 그 안전성 여부에 관한 판단과 사용에 관한 조치가 달랐다. 2004년 식약청은 1년여 간의 역학조사를 한 뒤, 비록 명백한 인과관계가 인정되지는 않았으나 pseudoephedrine이라는 대체약물이 있으며 국민들의 불안감을 해소시킬 필요가 있음 을 고려하여 PPA의 사용을 전면 금지하는 결정을 내렸다. WHO에 2005년 4월까지 보고된 자료에 의하면 PPA에 대해 어떠한 형태로 든 사용 제한 조치를 취한 나라는 13개국에 불과하고 이 13개국 간에도 조치 내용이 나라별로 상이하였다. 이로부터 우리는 기본적인 문제, 즉 왜 어떤 나라에서는 판매가 금지된 약물이 다른 나라에서는 판매될 수 있는가, 만약 약물의 안전성 판정기준이 오직 과학이 라고 한다면 PPA의 사용에 관한 제한 내용은 나라에 관계없이 일정해야 하는 것은 아닌가 등의 의문과 만나게 된다. 나라마다 인종 적∙유전적 차이 등이 있을 수 있지만, 약물의 안전성에 관한 판단기준이 오직 과학이라고 한다면 동일한 과학적 데이터의 해석과 약 물의 사용 제한에 관한 결정은 나라마다 합리적 일관성이 있어야 할 것이다. 따라서 관련 국제기구와 과학자들은 과학적 증거뿐만 아 니라 사회문화적 인자까지도 정량적으로 고려할 수 있는“의사결정 알고리듬”을 개발하는 데 협력을 아끼지 말아야 할 것이다. 지금 당장은 사회문화적 인자를 정량적으로 측정하고 판단할 수 있는 수준에 이르지 못하고 있지만, 약물유전체학의 발달로 과거 사회문화 적 인자라고 인식되던 약물반응의 인종 차이가 과학적 증거로 변환될 수 있게 된 것처럼 과학의 발달은 이러한 문제를 극복하는 데 커 다란 기여를 할 수 있으리라고 생각한다. 어떤 약물의 안전성에 대한 결론이 나라와 환경에 따라 불합리하게 달라지는 일을 막아야 할 것임은 자명하다는 점을 생각할 때, 의사결정 알고리듬의 개발을 위한 국제기구들의 협력은 더욱 절실히 필요하다 할 것이다. 나라에 따라 어떤 약물에 대한 안전성 평가와 그에 따른 사용 제한 조치 등이 불합리하게 달라진다면, 일반 국민들은 약물의 안전성과 관련된 규제기관의 결정을 신뢰하기 어려울 것이며 결국 약물도 신뢰를 잃게 될 수 있다. 이는 인류의 복지 향상을 위하여 약물의 가치를 극 대화하고 그 안전성에 대한 신뢰를 확보해야 하는 시대적 요청에 비추어 볼 때 매우 심각하게 대책을 마련해야 할 사항이라고 생각한 다. 또한 한 나라의 규제기관이 특정 약물의 안전성에 대하여 어떠한 결론을 내렸을 때 그 결론을 보건전문인이나 일반 국민에게 전파 하는 일에도 국제조화를 도모해야 할 것이다. 실제로 특정 국가나 지역에서는 판매 금지된 약물이 금지조치 이후에도 상당기간 유통되 는 사례가 많이 발견되고 있는데, 진정으로 그 약물이 안전하지 않은 것으로 판명되었다면 국제기구는 그 결정을 신속하고 완벽하게 전 세계에 전파하는 방법을 마련하기 위해 더욱 노력해야 할 것이다. 약물감시와 관련하여 국제적 통용이 가능한 합리적인 의사결정 알고리듬의 개발은 국제적 차원에서 약물의 위험에 대한 노출을 최소화하고 약물의 안전성 평가 및 관리상의 시간과 비용을 감소시키 는 일 등을 가능하게 함으로써 약물감시의 국제조화 및 인류의 보건복지 향상에 기여하는 중요한 역할을 담당할 수 있을 것이다.
