생명을 살리고 건강을 회복시킬 수 있는 최후의 의료 수단으로 간주되고 있는 장기이식을 위해서는 장기기증이 필수적이다. 장기기증의 필수 조건은 뇌사자 가족의 동의인데 이는 장기구득 면담을 통해서 이루어진다. 그러므로 장기기증 동의율 제고를 위해서 필요한 조건 가운데 하나는 장기구득 코디네이터의 면담 수행 능력을 향상시키는 것이다. 이를 위해서 무엇보다도 필요한 것은 효율적인 장기구득 면담의 조건과 요인에 대한 지식이다. 이 논문의 목적은 장기기증에 필수적인 뇌사자 가족의 장기기증 동의율 제고를 위해서 필요한 조건과 요인을 질적 연구를 토대로 밝혀내는 것을 목적으로 한다. 효율적인 면담의 조건과 요인의 탐구를 위한 자료는 다년간의 장기구득 면담 경험이 있는 코디네이터 10명을 대상으로 내러티브 인터뷰를 통해서 수집하였다. 연구 자료는 질적 연구 방법을 통해서 분석하였다. 연구 결과 장기구득 코디네이터가 생각하는 효율적인 뇌사자 가족과의 면담을 위한 조건과 요인은 모두 22개로 확인되었다. 이들은 의료진, 코디네이터, 가족 그리고 사회라는 대범주로 재분류되었다. 의료진 관련 조건과 요인으로는 가족에게 뇌사에 대한 충분한 설명이 있고, 코디네이터 관련 조건과 요인으로는 자질과 성향(경험과 자신감, 심리적 안정, 타자성, 장기기증의 의미에 대한 확신, 열정과 헌신, 여유와 유연함), 역량과 수행 능력(충분한 지식, 의사소통 능력, 경청과 공감 능력, 이해 능력, 관계형성 능력), 충분한 사전 준비(최대한의 정보 수집, 면담 계획 수립과 예행 연습, 공감과 지지를 위한 물품 준비) 및 환경 조성(독립적 면담 장소 확보, 면담 공간 분위기 조성, 충분한 면담 시간 확보) 등이 있다. 뇌사자 가족 관련 조건과 요인으로는 뇌사에 대한 이해와 수용, 장기기증에 대한 긍정적 인식, 심리적 안정 등이 있는 것으로 밝혀져다. 사회 관련 조건과 요인으로는 장기기증에 대한 긍정적 이미지가 있다. 이 연구의 결과는 장기구득 코디네이터의 면담 수행 능력 계발을 위한 교육과 다양한 교육 프로그램을 개발하는 데 기초 자료로 활용될 수 있을 것이다.
The consent of the deceased's family for organ donation is decided in organ procurement interview. Therefore, one of the necessary conditions to increase the organ donation consent rate is to improve the interview performance of the organ procurement coordinator. Knowledge about the conditions and factors of an effective organ procurement interview is essential to improve the interview skill of the organ procurement coordinator. The purpose of this paper is to find out, based on qualitative research, the conditions and factors necessary to improve the organ procurement interviews of the coordinator and to increase the organ donation consent rate of the brain-dead patient’s family. The data for exploring the conditions and factors of an effective interview were collected through narrative interviews with 10 experienced organ procurement coordinators. The research data were analyzed through a qualitative research method. As a result of the analysis, 22 conditions and factors for an effective interview with the brain-dead patient’s family were found. They were classified into four categories. Medical staff-related conditions and factors include a sufficient explanation of brain death to the family. Coordinator-related conditions and factors include qualities and dispositions (experience and confidence, psychological stability, otherness, confidence in the meaning of organ donation, passion and dedication, flexibility), competence and performance (sufficient knowledge, communication skills, active listening and empathy, comprehension ability, and relationship building skills), and sufficient preparation (collecting as much information as possible, planning and rehearsing interviews, preparation of supplies for support) and appropriate environment creation (private interview space, comfortable atmosphere for the interview, sufficient interview time). It was found that the conditions and factors related to the family of the brain dead patient include understanding and acceptance of brain death, a positive perception of organ donation, and psychological stability. As a social condition and factor, there is a positive image of organ donation. The results of this study can be used as basic data for developing various educational programs and education for improving the interview skills of organ procurement coordinators.
