연구 목적: 이 연구는 전라남도 거주 선천기형 환자의 수도권 지역 의료이용에 영향을 주는 요인을 규명하여 선천기형 환자의 의료이용에 대한 지역적 의료이용 형평성을 향상시킬 수 있는 방안을 제시하는 것이다. 연구 방법: 2016부터 2020년까지의 퇴원손상심층조사 자료를 이용하여 분석대상자의 수도권 지역 의료이용에 미치는 요인을 파악하기 위해 SPSS 27.0 통계 프로그램을 사용하여 빈도분석, 카이제곱검정과 로지스틱 회귀분석을 실시하였다. 연구 내용: 선천기형 입원환자의 수도권 의료이용 차이는 나이, 거주지역, 입원경로, 병상규모에서 유의한 차이가 났으며, 수술유무 및 중증도와도 유의한 차이가 있었다. 결론 및 제언: 선천기형 환자의 수도권 지역 의료이용의 형평성을 향상시키기 위해서 의료기관 및 병상수 공급보단 의료의 질적 수준을 향상할 수 있는 보건의료계획 및 정책이 수립되어야 할 것이다.
This study aims to identify the factors affecting medical use in metropolitan region by patients with congenital malformities in the Jeollanam-do region and suggest methods to improve equity in regional healthcare utilization. The study used Korean National Hospital's in-depth injury survey data from 2016 to 2020. Frequency analysis, the chi-square test, and logistic regression analysis were conducted using SPSS 27.0 Statistics to identify the factors affecting the patients’ utilization of metropolitan hospitals. The utilization by their general characteristics was significantly different in age, residential area, admission routes, bed size, surgical process presence, and severity level. Effective regional healthcare planning and policy should be established for patients with congenital malformities to improve the quality of medical care over the supply of medical institutions and hospital beds.
