연구목적: 국내 성인을 대상으로 인플루엔자 고위험군에 속하는 만성 질환자에게서의 접종 현황을 알아보고, 접종에 영향을 미치는 요인을 분석함연구방법: 본 연구는 제 7기 국민건강영양조사(2016~2018년) 자료를 이용하였으며, 복합표본분석을 실시하였다. 만 19세 이상 성인 중 결측치가 없는 17,374명을 대상으로 인플루엔자 예방접종 현황을 살펴보았으며, 그 중 만성 질환자 5,650명을 대상으로 예방접종에 영향을 미치는 요인을 평가하였다. 분석은 SAS 9.4를 활용하여 기술통계와 빈도분석, 로지스틱 회귀분석, Rao-Scott 카이제곱검정을 실시하였다. 주요결과: 전체 성인의 인플루엔자 백신 접종률은 37.1%였다. 만성 질환이 있는 사람은 그렇지 않은 사람보다 접종률이 더 높게 나타났으며, 만성 질환자 중 65세 미만에서는 38.1%, 65세 이상에서는 86.0%로 나타났다. 무료 백신 접종대상인 만 65세 이상을 제외하고 분석하였을 때, 기혼인 사람은 2.28배, 의료급여대상자는 2.23배 높은 접종률을 보였다. 한편, 가족 구성원 중 미성년자가 있는 사람에게서의 접종률은 0.75배 낮게 나타났다. 결론: 국가예방접종사업의 대상이 되는 만 65세 이상과 지자체의 무료 인플루엔자 백신 접종 대상이 되는 의료급여 대상자에서 높은 접종률이 나타났으며, 이를 바탕으로 무료접종은 인플루엔자 예방 접종에 가장 주요한 원인으로 볼 수 있다.
Purpose: This study is aimed to identify the influenza vaccination rate and to estimate the effect with chronic diseases in the high risk group of adults in Korea. Method: Using the data from the 7th Korea National Health and Nutrition Examination Survey(KNHANES Ⅶ), this study is conducted by a complex sample analysis. The influenza vaccination coverage was determined in adults aged 19 years or older. Among them, factors affecting immunization were identified in 5,650 patients with chronic diseases. The analysis was performed using logistic regression, and Rao-Scott chi-square test. Results: The influenza vaccination coverage in adults was 37.1%. Compared to those without chronic disease, patients with chronic disease reported higher vaccination rate. Among those with chronic diseases, the vaccination rate was 38.1% for under 65 and 86.0% for over 65. When analyzed, excluding those aged 65 or over, who were eligible for free vaccination under the National Immunization Program (NIP), the coverage of vaccination was 2.28 times higher in married people and 2.23 times higher in Medical aid program. On the other hand, the vaccination rate in people with minors among family members was 0.75 times lower. Conclusion: The beneficiaries of NIP and medical aid programs received a free vaccine and had high immunization rates. This indicated that free vaccination can be considered as the main factor of influenza vaccination. On the basis of this, free vaccination can be considered as the main cause of vaccination.
