본 연구는 연하기능강화와 응급상황대응 교육프로그램이 자기효능감과 수행능력에 미치는 효과를 규명하고자 시도된 비동등성 대조군 전후 유사 실험연구이다. 연구대상은 서울소재 재가노인복지센터에서 가정방문 서비스를 제공하는 요양보호사 56명이다. 자료수집은 2014년 2월부터 2014년 4월까지로 실험군은 연하기능강화와 응급상황대응 교육프로그램을 실험 첫 주와 4주 후 각각 16시간을 제공한 후 자기효능감은 설문지를 통해 자기보고식으로 측정하고 수행능력은 연구보조원에 의해 평가하였다. 자료분석은 SPSS Window 20.0 version을 이용하여 분석하였다. 일반적 특성은 실수, 백분율, 빈도, 평균, 표준편차를 실시하였고, 실험군과 대조군의 동질성 검정은 -test와 t-test를 연하기능강화와 응급상황대응 교육프로그램의 두 군간 시간경과에 따른 종속변수의 변화와 그룹과 시간과의 교호작용에 대한 검증은 two-way repeated measure ANOVA로 분석하였다. 연구결과 연하기능강화와 응급상황대응 교육프로그램을 제공받은 실험군은 연하기능강화와 질식대응, 심폐소생술의 자기효능감과 수행능력에 대한 반복측정분산분석결과 초기교육 직후 점수가 상승하였고 초기교육 4주 후에는 자기효능감과 수행능력의 점수가 감소하였으나 재교육 4주 후 다시 상승하여 시간에 따라 자기효능감과 수행능력 점수의 변화가 유의하게 차이가 있는 것으로 나타났다. 또한, 집단과 측정시점 간의 교호작용도 유의한 차이가 있는 것으로 나타나 연하기능강화와 응급상황대응 교육프로그램이 재가방문요양보호사의 자기효능감과 수행능력에 효과가 있음을 확인하였다.


This quasi-experimental study aimed to investigate the effect of education program for improvement of swallowing ability in proper management of dysphagia swallowing ability swallowing function and emergency response on self-efficacy and performance of home visiting care workers who provide in home supportive services such as personal care services and bathing at home. Fifty-six home visiting care workers at two community care centers for the elderly in Seoul were enrolled in this study. From February, 2014 to April, 2014, data was collected on weekends considering work schedules of them. The experimental group received the education program for improvement of swallowing ability in proper management of dysphagia and emergency response for 16 hours initially during the first week in this study. Four weeks later, 16 hours of re-education was provided additionally. In order to investigate baseline characteristics and self-efficacy, structured questionnaires were completed by subjects themselves. A research assistant who had not been involved in the education program evaluated their performance using the observational method. Data was analyzed using SPSS Window 20.0 version. Baseline characteristics were analyzed by real numbers percentages frequency mean and standard deviation and -test, t-test was used to analyze the homogeneity of experimental and control group. In addition, changes in outcome variables of education program over time between two groups and the test for interaction with time were analyzed by two-way repeated measure ANOVA. As a result of this study, there was a statistically significant difference in self-efficacy and performance scores for improving between the experimental group and the control group. Also, there was a significant difference in measuring time and in the interaction between groups and measuring time. In conclusion based on this study it was turned out that the effects of clinical application of education program for home visiting care workers.
