마비말장애 환자들의 조음 속도와 모음 음향 공간에 대한 연구를 위하여 특발성 파킨슨병으로 확진을 받았으나 약물중재 전 상태인 환자를 대상으로 약물중재 효과를 배제하고 병의 유무만으로 조음 속도와 모음 음향 공간과의 상관을 연구하고자한다. 대상자는 특발성 파킨슨병 환자군 여성 10명(평균 연령 64.8±7.2세)과 통제군 여성 10명(평균 연령 66.5±6.1세)이며 조음 속도와 관련하여 5초간 SMR(/퍼터커/)과 AMR(/퍼/,/터/,/커/) 횟수를 측정하였고, 모음 음향 공간의 특성을 살펴보기 위해서 문두(sentence initial)에 위치한 모음 /아, 이, 오/ 안정구간의 포먼트 값을 추출하였다. 분석 수치는 모든 파라미터 3~4회의 평균값을 사용하였고, 통계분석은 SPSS 12.0을 이용하여 5% 유의수준에서 비모수 검정인 Mann-Whitney U 검정과 Pearson 상관분석을 실시하였다. 연구결과, 첫째, 조음 속도에서 환자군이 통제군보다 낮은 수치를 보였다. 둘째, 음향공간 내의 모음 분산도에서 환자군이 통제군보다 넓게 산포되었다. 셋째, 환자군이 통제군보다 유클리드 거리가 짧게 측정되었다. 넷째, 모음 음향 공간의 면적에서 환자군이 통제군보다 좁게 나타났다. 따라서 퇴행성 질환으로 인한 마비말장애 환자들의 언어치료로 조음 음향 공간 확장 연습이 필요하다.
This study sought to characterize the articulatory rate and acoustic vowel space of people with dysarthric idiopathic Parkinson's disease before pharmacological intervention. Ten female patients with idiopathic Parkinson's disease and a healthy control group of 10 females were selected and every voice was recorded 3-4 times using praat (ver. 5022) with a headset microphone. Parameters of the articulatory rate were sequential motion rate (/pʌtʌkʌ/) and alternating motion rate (/pʌ/,/tʌ/,/kʌ/) in five second intervals. The acoustic space parameters for vowels were as follows: Euclidean distance between vowels (/a/_/i/, /i/_/o/, and /o/_/a/) and acoustic space area. All parameters were statistically analyzed by Mann-Whitney U-test and Pearson correlation analysis. The results were (1) The articulatory rate of the patient group was lower than that of the normal group. (2) The Euclidean distance and vowel space area were smaller for the patient group than the normal group. (3) There were positive correlations among three parameters of articulatory rate in the normal group. However there was a positive correlation only of [AMR/pʌ/ & AMR/tʌ/] in the patient group. In particular, only the patient group showed a positive correlation between SMR and I_O distance. These findings indicate that Parkinson's disease causes a decrease of mandible movement. This physical change has the characteristics of dysarthric articulation. Consequently, in speech therapy sessions for regressive dysarthria patients, drills for articulatory acoustic space extension are important.
