담도계의 악성 질환이 있을 때 내시경적 역행성 담도 배액술 등의 비수술적 담관 배액술을 시행한다. 이 때 협착 부위의 확장은 일반적으로 확장용 카테터나 풍선 카테터를 사용하여 확장한다. 그러나 중증의 협착은 일반적인 확장 방법으로는 쉽게 확장되지 않는다. Soehendra 배액관 제거기는 담도 내에 파묻힌 배액관을 제거하는 데 효과적인 기구이다. 하지만 최근에는 중증의 악성 담도 협착의 확장에 Soehendra 배액관 제거기를 사용하고 있다. 저자 등은 중증의 악성 담도 협착의 환자에서 Soehendra 배액관 제거기를 사용하여 배액술을 성공시킨 3예를 보고하는 바이다.
Malignant biliary or pancreatic duct stricture is dilatated using dilatating catheter or balloon catheter but a high grade stricture is difficult to dilate by general dilatating methods. Using Soehendra stent retriever is an effective method for exchanging an impacted stent. Recently, high grade malignant biliary strictures have been dilatated by Soehendra stent retriever. We report 3 cases of successful biliary drainage in high grade malignant biliary strictures using 7 Fr Soehendra stent retriever. (Korean J Gastrointest Endosc 2002;24:62-65)
