목적 : 안구 둔상 후에 발생한 저안압 망막병증을 유리체강내 가스주입술로 치료한 예를 경험하였기에 보고하고자 한다. 대상과 방법 : 내원 전 못에 의해 좌안에 둔상을 입은 후 갑자기 발생한 시력저하를 주소로 38세 남자 환자가 내원하였다. 내원 당시 좌안시력은 0.06, 안압은 1 mmHg이었고, 외안부 열상 소견은 발견되지 않았다. 전방출혈, 경도의 황반부종과 얇은 선상의 황반 주름을 동반하였고, 앞방각경 검사상 모양체해리 소견이 관찰 되었다. 내원 당일 유리체강내 가스(C3F8)주입술을 시행하고 경과를 관찰하였다. 결과 : 유리체강내 가스주입술 시행 후 1일째 안압은 정상화 되었고, 7일째 좌안시력 1.0을 회복하였다. 술 후 10개월 동안의 추적 관찰 기간 동안 정상 안압과 시력을 유지하였고 외상에 의한 합병증은 관찰되지 않았다. 결론 : 모양체해리를 동반한 저안압 망막병증에서 유리체강내 가스주입술은 간단하면서도 안전하므로 보다 침습적인 치료에 앞서 고려될 수 있는 좋은 치료 방법이라 생각된다. <한안지 47(2):319-322, 2006>


Purpose: We report a case of post-traumatic hypotony retinopathy with cyclodialysis cleft treated by intravitreal gas injection. Methods: A 38-year-old man presented with decreased visual acuity after blunt trauma to the left eye. Visual acuity of the left eye was 0.06 and intraocular pressure was 1 mmHg. No external wound was found. The anterior chamber showed a microscopic hyphema and, upon funduscopic examination, mild macular edema and striae were seen. The cyclodialysis was identified using gonioscopy. We treated the patient with intravitreal gas (perfluoropropane, C3F8) injection under topical anesthesia and tracked the outcome of this treatment. Results: On the first postoperative day, intraocular pressure fell to the normal range. After 7 days the visual acuity improved to 1.0. During a follow-up period of 10 months, intraocular pressure and visual acuity remained in their normal range. Conclusions: Intravitreal gas injection offers a safe and effective method of treating hypotony retinopathy with cyclodialysis cleft. J Korean Ophthalmol Soc 47(2):319-322, 2006
