목적 : 국내에서 처음으로 시행된 보스톤 1형 인공각막이식술의 두 증례를 소개하고자 한다. 대상과 방법 : 양안의 화학화상으로 시각상실 발생하였던 환자와, 수상에 의한 각막혼탁으로 2회의 동종각막이식술을 받았으나 모두 이식거부가 발생하였던 환자를 대상으로 하였다. 보스톤 1형 인공각막은 front part, back plate, locking ring으로 구성되어 있다. 수술 방법은 8.5 mm 크기로 원형절제된 공여각막의 중심부에 3 mm 크기로 구멍을 뚫은 후, 공여각막의 앞쪽으로 front part를 놓고 front part의 stem 부위를 공여각막의 중심부 구멍을 통해 통과시키고, 공여각막 뒤쪽으로 back plate를 stem에 끼워 맞춘 후, locking ring으로 고정하여 이식편을 만들었다. 이후 환자의 각막을 8 mm 크기로 원형절제 하였고, 준비된 이식편을 직접 봉합하였다. 결과 : 두 증례에 대해 성공적으로 보스톤 1형 인공각막이식술이 시행되었다. 증례 1은 기존의 시신경장애와 유리체출혈의 발생으로 시력예후는 불량하나 미용상 효과는 좋았고, 증례 2는 술후 3개월에 나안시력 0.1을 유지하고 있다. 결론 : 앞으로 국내에서 많은 수의 수술 증례와 오랜 기간의 경과 관찰로 본 술식의 안전성 및 유효성에 대한 연구가 기대된다.

Results: Two Boston (Dohlman-Doane) Type Ⅰ Keratoprosthesis procedures were performed successfully. The first case had poor visual prognosis due to preexisting optic neuropathy and vitreous hemorrhage, but was showed better for cosmetic effect. The second case had maintained an uncorrected visual acuity of 0.1 at postoperative 3 months. Conclusions: We hope to perform additional procedures and long-term follow-ups to further evaluate this new surgical technique for stability and efficiency.
