목적 : 본 병원에서 경험한 비전형적인 형태의 테리엔변연각막변성을 보고하고자 한다. 대상과 방법 : 59세의 여자가 2004년 우안의 심한 이물감을 주소로 내원하였다. 11년전인 1993년 이물감을 주소로 본원 안과를 처음 방문하였으며, 테리엔변연각막변성 진단하에 간헐적으로 외래 추적을 받았다. 초진 당시 나안 시력은 우안 1.0 좌안 1.0 이었으며 세극등 검사에서 양안 모두 주변부 각막이 360°에 걸쳐 고리모양으로 얇아져 고랑을 이루고 있었다. 11년 동안 전반적인 형태는 진행하지 않고 그대로 유지되고 있었으나, 최근 외래 내원시 석회화된 지방축적물이 상피 표면 밖으로 돌출되어 있는 것을 관찰할 수 있었다. 결과 : 본 증례는 다른 테리엔변연각막변성과는 달리 고랑과 지방침착이 각막 둘레를 모두 침범하여 360°에서 지방침착물이 동반되어 있는 점이 특이 소견이며, 또한 10여년이 지나면서 지방 침착이 축적되어 각막 상피층을 파괴하고 실질 밖으로 돌출되어 있는 점은 현재까지 보고된 증례와는 다른 비전형적인 형태이기에, 문헌 고찰과 함께 보고하는 바이다.


Purpose: We report an atypical case of Terrien’s marginal degeneration with epithelial defect and peripheral 360°lipidopathy. Methods: A 59-year-old female patient visited our clinic complaining of severe foreign body sensation in her right eye. She had visited our clinic with foreign body sensation 11 years before. She has been followed-up for 11 years with a diagnosis of Terrien's marginal degeneration. Upon ocular exmaniation, visual acuity of both eyes was 20/20. Both eyes had 360°circumferential peripheral thinning with deposition of lipids. The lesion progressed very little over past 11 years. For the patient's most recent examination, the foreign body sensation is believed to be caused by calcified lipid deposition protruding from the corneal stroma with an epithelial defect. Results: What makes this case unique is the 360°circumferential peripheral thinning with deposition of lipid and calcification of lipid deposition which protruded from corneal stroma with an epithelial defect.
