목적 : 보크트-고야나기-하라다 증후군(Vogt-Koyanaki-Harada syndrome) 환자 1예에서 치유경과에 따른 망막기능의 변화를 다국소 망막전위도를 이용하여 알아보고자 하였다. 대상과 방법 : 보크트-고야나기-하라다 증후군으로 진단받은 32세 여성에서 치료 전, 치료 3개월, 치료 6개월의 망막변화를 형광안저혈관조영, 망막전위도, 다국소 망막전위도를 이용하여 비교하였다. 결과 : 시간이 지남에 따라 시력이 회복되었으며 후극부의 장액성 망막박리가 회복되면서 형광안저혈관조영에서 후기 망막하누출이 소실되었고 망막색소상피의 응집 및 위축을 보였다. 망막전위도에서는 초기에 반응이 감소하였으나 점차로 회복되어 6개월째 정상 수준의 반응을 보였다. 다국소 망막전위도는 치료 전에 비해서 치료 6개월 후 반응이 증가되었으나 여전히 국소적으로 반응 결핍이 관찰되었다. 결론 : 본 환자로 미루어 보아 후극부의 장액성 망막박리가 회복되어도 망막기능의 이상은 지속되는 것을 다국소 망막전위도를 통해 알 수 있었으며 이 질환의 환자들에 대해서 다국소 망막전위도의 장기간 체계적인 검사가 필요하고 이에 따른 적절한 치료가 필요할 것으로 생각된다. <한안지 47(3):494-499, 2006>


Purpose: To assess the changes of retinal function together with recovery progress in a patient with Vogt-Koyanaki-Harada syndrome using a multifocal electroretinogram (mfERG). Methods: The retinal changes of a 32-year-old woman diagnosed with Vogt-Koyanaki-Harada syndrome have been investigated and compared for different times such as before treatment and 3 and 6 months after treatment using a fluorescein angiogram, electroretinogram (ERG) and mfERG. Results: As time passed, the patient's visual acuity was recovered, the serous retinal detachment in the posterior pole was restored, and the phenomenon of late subretinal leakage disappeared on fluorescein angiogram, while aggregation and atrophy of the retinal pigment epithelium were detected. The early-stage responses of the retina decreased on ERG; however, it has gradually recovered and was determined to be normal after 6 months. Responses measured on the mfERG have increased during the 6 months after surgery compared to the pre-treatment, but there was still a partial lack of response. Conclusions: In this case, the abnormal condition of retinal function existed continuously on the mfERG, even when the serous retinal detachment in the posterior pole was recovered. In this regard, a long-term systematic investigation and subsequent mfERG treatment have been thought to be necessary for patients with this disease. J Korean Ophthalmol Soc 47(3):494-499, 2006
