목적: 정상안압녹내장 환자의 초진시 중심각막두께와 망망신경섬유층두께와의 상관관계 및 중심각막두께에 따른 망망신경섬유층 손상 진행정도의 차이를 알아보고자 한다. 대상과 방법: 정상안압녹내장으로 진단, 약물치료 중인 36명 51안과 정상인 30명 51안을 대상으로 안압, 중심각막두께 측정, 빛간섭단층촬영을 이용한 망막신경섬유층두께를 측정하여 초진 시 중심각막두께와 망막신경섬유층 두께 사이의 상관관계를 알아보았고 정상안압녹내장군 중 3년 동안 3회 이상 빛간섭단층촬영을 시행한 환자를 얇은 중심각막두께군(553.6 μm >)과 두꺼운 중심각막두께군(553.6 μm ≤)으로 나누어 두 군의 망막신경섬유층 두께의 변화를 비교하였다. 결과: 초진 시 정상안압녹내장군에서 중심각막두께와 망막신경섬유층두께 사이에 유의한 상관관계가 있는 것으로 나타났고(R=0.656,p<0.05), 망막신경섬유층의 손상과 중심각막두께가 얇은 군과 두꺼운 군 간에 유의한 차이를 보이지 않았다(p>0.05). 결론: 정상안압녹내장 환자의 초진 시 중심각막두께와 망막신경섬유층두께 사이에는 유의한 양의 상관관계를 보였으나 망막신경섬유층손상의 진행과는 유의한 결과를 보이지 않았다.


Purpose: To investigate the correlation between central corneal thickness (CCT) and peripapillary retinal nerve fiber layer (RNFL)thickness in the eyes of patients with normal tension glaucoma (NTG) at the initial examination and to examine the difference in the degrees of damage of RNFL thickness depending on the CCT. Methods: Fifty-one eyes of 36 patients with NTG were included in the study, and 51 eyes of 30 people without the disease were used as a control group. CCT and peripapillary RNFL thickness were measured in each eye by ultrasonic pachymetry and optical coherence tomography(OCT), respectively. Patients from the normal NTG group who underwent OCT more than three times inthree years were identically assigned to two groups based on CCT: thin (< 553.6 μm) and thick (≥ 553.6 μm). Thus, a comparison of the changes in the thickness of retinal nerve fiber layer was performed between the two groups. Results: Patients with NTG showed a significant positive correlation between CCT and the mean peripapillary RNFL thickness at the initial examination (R=0.68, p<0.01). There were no significant differences in the changes in thickness of RNFL between the groups based on CCT (p>0.05)Conclusions: At the time of initial diagnosis of NTG, there was a significant correlation between the thickness of CCT and RNFL. Howeverthere was no significant correlation between the thickness of CCT and the progression of the damage of RNFL.
