목적: 여과수술 후 0.2% 사이클로스포린 점안 효과를 동물실험을 통해 알아보고자 하였다. 대상과 방법: 토끼 8마리 16안에 후공막절제술 시행 후 0.2% 사이클로스포린을 우안에 1일 2회, 좌안에 피마자유만 점안하였다. 술후 1, 3, 5일과 1, 2, 4 주에 안압을 측정하였고, 술 후 1, 2, 4, 8주에 두 마리씩 희생하여 hematoxylin & eosin (H&E) 염색과 bromodeoxyuridine (BrdU) 면역염색을 시행하여 증식하는 결막하 섬유모세포를 관찰하였다. 결과: 1, 2주에서 안압은 실험군이 각각 15.44±2.74, 15.00±2.74 mmHg으로 18.06±3.17, 16.83±2.21 mmHg인 대조군보다 유의하게낮았으며(P=.047, P=.48), 면역염색에서 술 후 1주째 증식하는 섬유모세포의 비율도 실험군 20.17±1.37%, 대조군 30.83±0.22%로유의한 차이가 나타났다(P=.003). 결론: 0.2% 사이클로스포린을 점안 시 술 후 초기에 섬유모세포의 증식과정을 억제함으로써 창상치유 과정을 변화시키는 것을 관찰하였다. 향후 저농도 사이클로스포린 보조요법에 대한 가능성을 제시한 것으로 사료된다.


Purpose: The effect of 0.2% cyclosporin A (CsA) as an adjuvant therapy after glaucoma-filtering surgery was the focus of this study. Methods: A posterior lip sclerotomy was performed in 16 eyes of 8 rabbits, and 0.2% CsA was administered into the right eyes. The left eyes served as controls. The intraocular pressure (IOP) was measured 1, 3, 5, 7, 14, and 28 days after surgery. Hematoxylin-eosin (HE) and anti-bromodeoxyuridine (BrdU) immunocytochemical staining were performed at 1, 2, 4, and 8weeks. Results: The IOP at 7 and 14 days after surgery was lower in the 0.2% CsA group and statistically significant (P=0.047, P=0.48;respectively). HE staining did not show any difference between experimental and control eyes, but anti-BrdU staining showed a lower number of positive cells in the experimental eyes at 1 week. The fibroblast proliferation rate was significantly lower 1 week after surgery in the 0.2% CsA group (P=0.003). Conclusions: An effect of 0.2% CsA on early wound healing was observed. The data suggest that a low concentration of CsA can be useful when employed as adjuvant therapy in glaucoma filtering surgery.
