일차성 쇼그렌 증후군에서 중추 신경계 침범은 빈도가 낮으며 그 중 횡단 척수염 동반은 드물게 보고 되었다. 저자 등은 양하지 저린감과 배뇨 장애를 주소로 내원한 환자에서 급성 횡단 척수염으로 발현된 일차성 쇼그렌 증후군으로 진단하고 고용량의 methylprednisolone과 면역 억제제를 사용하여 호전된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
Sjögren's syndrome is one of chronic inflammatory autoimmune diseases involving exocrine gland. Involvement of central nerve system was rarely reported. We report a case of primary Sjögren's syndrome presenting as transverse myelitis in 37-year-old female who complained of tingling sense in both legs and voiding difficulty. Magnetic resonance imaging shows high signal intensity with cord swelling from C6 to T7 in T2 weighted image. The patient received intravenous methylprednisolone (1,000 mg/day) for 10 days followed by prednisolone and intravenous monthly cyclophosphamide with subsequent clinical improvement.
