목적: 고관절 주위 석회화 건염의 임상 양상과 보존적 치료 방법중 비스테로이드 소염제 경구 투여와 부신피질 호르몬과 국소 마취제의 혼합 주사 치료 효과에 대해 비교 연구 하였다. 대상 및 방법: 고관절 주위 석회화 건염으로 치료 받았던 9례에 대하여 후향적 분석을 하였으며, 7례에서 자기 공명 영상을 시행하였다. 치료 방법은 수술적 제거 1례, 비스테로이드 소염제 경구 투여 4례, 부신피질 호르몬과 국소 마취제를 혼합 주사 치료한 경우가 4례였다. 결과: 비스테로이드 소염제 경구 투여로 치료한 4례에서 증상 소실에 필요한 시간은 2주 이상(2주-6개월)이 소요된 반면 부신피질 호르몬과 국소 마취제를 혼합 주사한 4례 및 수술적 제거를 시행한 1례에서는 즉각적인 증상 호전이 나타났다.모든 예에서 재발은 없었다. 결론: 고관절 주위의 통증이 심한 경우 석회화 건염을 의심해야 하며, 부신피질 호르몬과 국소 마취제의 혼합 주사 치료도 좋은 치료 방법으로 사료된다.
Purpose: The clinical symptoms of calcific tendinitis around the hip joint were reviewed, and the effect of oral nonsteroidal anti-inflammatory agents compared with that of a mixed injection of corticosteroid and local anesthetics as conservative treatments. Materials and Methods: Nine cases were retrospectively reviewed in this study. Magnetic resonance imaging was performed in seven cases. One case was treated with surgical excision, four with oral nonsteroidal anti-inflammatory agents and four with the mixed injection of corticosteroid and local anesthetics. Results: The four cases treated with the mixed injection of corticosteroid and local anesthetics had immediate relief of pain, but the four treated with the nonsteroidal anti-inflammatory agents took more than 2 weeks to achieve pain relief. There was no recurrence in any of the cases. Conclusions: Dramatic relief of pain was noted in all cases treated with the mixed injection of corticosteroid and local anesthetics.
