정신과전공의 수행평가는 비교적 최근 시작되어 2009년에 제5회 시험을 시행하였으며, 향후 새로운 발전 및 변화가 필요하다는 논의가 이루어지고 있다. 이에 대한신경정신의학회 수련위원회는 지난 5년간의 수행평가를 정리-분석하여 향후 논의를 위한 토대로 삼기 위해 이 논문을 작성하게 되었다. 논문 작성을 위해 지난 5년간의 수행평가 관련 공식자료를 취합하고, 종적 변화에 대한 자료를 새로이 분석하였으며 그 결과 다음 소견을 얻었다. 1) 연차별로 수행능력의 차이가 있어서, 고년차로 갈수록 성적이 향상되는 것이 뚜렷이 관찰되었다. 그런 경향은 특히 정신약물학-생물학적 기초, 노인 정신의학, 소아 정신의학 및 응급정신의학-기질성 장애 분야에서 잘 나타났다. 정신증 분야에서는 저년차와 고년차간 수행능력에 차이가 없었다. 2) 수련기관의 형태별로 수행능력의 일부 차이가 있어서, 불안장애-신체형 장애, 정신생리장애 및 노인정신의학 분야에서 대학병원이 정신과전문병원보다 높은 성적을 보여주었다. 3) 중복수험자의 성적 분석 결과, 2회의 성적 간에 연관성은 있지만, 연차가 올라갈수록 수행능력이 증가하는 경향도 관찰되었다. 이상의 결과는 정신과전공의 수행평가가 전공의 개인의 현재 수행능력 및 수련에 따른 능력 향상, 수련기관별 강점 및 취약성 등을 보여주는 지표로 활용가능 하다는 것을 보여주었다. 이를 통해서 전공의 개인은 및 수련기관 모두 자신의 취약점과 강점을 이해하고 보완함으로써 더 개선된 정신과 전공의 수련을 할 수 있으리라 기대한다.


Objectives: The in-training examination (Performance Examination, PE) for psychiatric residents in Korea was launched 5 years ago by the Korean Neuropsychiatric Association (KNPA). This article analyzes 5-year accumulated data on the PE, and tries to make some suggestions for further development of the PE. Methods: The 5-year data, previously utilized for the generation of formal annual reports were reanalyzed, with an emphasis on longitudinal trends. ResultsZZThe analyses indicated the following;1) Higher-year residents earned definitely higher scores than their lower-year colleagues on the PE. This trend was especially prominent in the area of psychopharmacology-biological psychiatry, geriatric psychiatry, child and adolescent psychiatry, and the emergency-organic psychiatry. There was no year-related performance difference in the area of psychoses. 2) In the area of anxiety-somatization disorder, psychophysiological disorder, and geriatric psychiatry, the residents in the university-affiliated hospitals outperformed those in the specialized psychiatric hospitals. 3) Through analyzing multiple-times examinees, it was found that their first-and second-time performances were moderately correlated, and that their ranks tended to improve, demonstrating a continuously improving performance according to the training year. Conclusion: These result suggested that the KNPA PE is a feasible measure for the estimation of an individual resident’s performance as well as the adequacy of the environment provided by the training institutes.
