The purpose of this study was to examine the relationships between the motivations and perceived service qualities of the medical tourism in Korea. Also the purchasing behaviors of medical tourists were used as a result variable. For this study, survey was conducted at Incheon International Airport from July 1 to August 31, 2011. Total 250 questionnaires were distributed and 208 were used for the analysis of this study. PASW 18.0 was used for statistical purpose. Casual step approach was used for examine mediated effect of service qualities between motivations and purchasing behaviors of medical tourists. Experience motivations of medical tourists effects positively on tangibility, reliability, and responsiveness of service quality. Medical expenses and service motivations of them effect positively on assurance and reliability of service quality. Assurance and reliability of service quality have positive impact on emotional behavior of medical tourists. The reliability and assurance of service quality of medical tourism have mediate effect between motivations and purchasing behaviors of medical tourists.
