Foix-Chavany-Marie syndrome (FCMS) is caused by bilateral frontoparietal opercular lesions, and there are loss of voluntary control of facio-pharyngo-lingual-masticatory muscle. A 59 year-old man was admitted due to sudden left hemiparesis and severe dysphagia. He could blink and yawn spontaneously, but he could not close eyes and open mouths on command. Five years earlier he had experienced hemorrhage in the left basal ganglia resulted in the right sided hemiparesis. Recovery from this previous stroke was nearly complete. Brain MRI showed lesions in the right anterior operculum and the left posterior internal capsule-globus pallidus. We report a patient with FCMS was caused by unilateral anterior opercular and contralateral internal capsule lesions.
